Description

We are looking for a passionate advanced clinical practitioner or an individual looking to develop into the role as a trainee advanced clinical practitioner.

Based within the Richmond Response and Rehabilitation Team (RRRT), the post holder will provide clinical expertise and practice leadership to the RRRT team and across Richmond unplanned and integrated community services in collaboration with medical, nursing and allied health professional colleagues and in recognition of knowledge and skills outside of their own clinical professional background.

They will ensure clinical staff are equipped, prepared and suitably trained in delivering person centred high quality out of hospital care.


  • Provide expert clinical and professional advice to support the multidisciplinary team within Hounslow, Trust and outside as appropriate
  • Work across organisational and professional boundaries, to develop effective partnerships working in accordance with agreed Trust, South West London and national strategic direction
  • Maintain an overview of the team caseload of patients; providing specific advice and support with complex patients and ensuring delivery to key standards and performance indicators to ensure an efficient and effective service.
  • Work as an expert practitioner demonstrating advanced knowledge and skills beyond traditional nursing and therapy roles, including the integration of research into practice.
  • Use expertise and decisionmaking skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidencebased judgements and/or diagnoses.
  • Exercise professional judgement to mitigate and manage risk appropriately
  • Act as a clinical role model, advocating change where required and delivering care that is responsive to changing need.

To provide responsive and expert clinical care for adult patients within the Community.


To lead and provide direction to the Richmond Response and Rehabilitation Team in the care of older adults with multiple chronic diseases with the aim of supporting discharge as well and prevention of hospital admission through advanced level autonomous practice including taking managed risks.

High levels of leadership skills and professionalism are required in this role.

To be a strong visible clinical leader working in partnership within a multi-disciplinary team.


To provide support and supervision to junior members of the team and any other professionals that work in the Richmond Response and Rehabilitation team.
