    Nursing Associate / Assistant Practitioner, Band 4

    The West South Somerset District Nurses are looking for a dynamic Band 4 Nursing Associate / Assistant Practitioner to join the teams who work across the Chard, Ilminster, South Petherton, Langport & Crewkerne Neighbourhood.

    We visit patients in their own homes and work along side our colleagues from the integrated teams, including CRS, OPMH, GPs, Complex Care Team Palliative care teams, Adult Social Care, village agents and Hospital at Home.

    We offer a integrated service to help our house bound patients. you will be part of a fantastic team who are multi skilled.

    Person Specification

    Qualifications & Training

    Essential

  • Level 2 English and Math's - (equivalent to GCSE grade C or above).
  • Demonstrates meeting the standards of the Care Certificate.
  • An accredited level 5 vocational qualification such as a Foundation Degree or other Level 5 Diploma in Healthcare.
  • Experience

    Essential

  • Substantial proven experience in health care.
  • Supervision of others.
  • Competency in the range of developmental competencies identified in Appendix 1.
  • Skills

    Essential

  • Car driver with access to own transport.
