Reablement Worker needed with London Borough of Hammersmith and Fulham

Salary - £19.45ltd per hour

Description:

As a reablement worker you will be supporting clients with personal and practical care tasks whilst maintaining and developing their social contacts.
To provide written and verbal reports regarding the activity undertaken and progress made against goals during supervision, at reviews and when required, utilising electronic and paper based records
To work in partnership with other teams and agencies within the borough to maximise reablement potential and support the transition into and out of the Community Independence Service.

What is required?

Previous experience of working with vulnerable Adults within the community
Educated to GCSE Level Grade A-C in Maths and English
