    As ourPrimary Care Network develops in Pendle East, we are looking for anexperienced, highly motivated Clinical Pharmacist to join our expanding Neighbourhood Team.

    Based in our Hub in Colne Health Centre together with other members of our Multi-disciplinaryteam, or in our GP Practices, you will be working with us to change the way primary care is deliveredin Pendle East.

    Main duties of the job

    You will have a minimum of 5 years post-registration experience, and will use your expert knowledge of medicinesto support our Primary Care Network in providing effective patient services to residents of Pendle East. You will also contribute to the continuedlearning of PCN colleagues.

    The role combines patient-facing, analytical and administrative elements of work working with our GP practices, as well as the opportunity to develop new ways of working across our wider Neighbourhood. It is an exciting time to be part of our team

    The role of PCN Pharmacist is already established within Pendle East PCN and we are now looking for an additional Pharmacist to join our expanding team.

    Based within the Pendle East Community Hub in Colne Health Centre, as well as the GP practices within our group, the post will join other members of our wider neighbourhood team in providing a growing range of services.

    The successful Pharmacist will work closely with our local GPs, existing practice and PCN Pharmacists and prescribing administrative support teams.

    Job responsibilities

    The duties of this post will be subject to regular review, at least on an annual basis. The duties will reflect the requirements from the PCN DES and the GP contract in relation to prescribing and medicines management. These will include but are not limited to:

    1. Clinically assess and treat patients using expert knowledge of medicines for specific disease areas.

    2. Undertake Structured Medication Reviews to ensure patients take their medications correctly and get the best from them, reducing waste and promoting self-care.

    3. Be a prescriber, or commit to completing training to become a prescriber.

    4. Contribute to the delivery of effective daily processes for the transfer of care between hospitals and GPs including effective signposting, querying discrepancies and recommending appropriate medication changes.

    5. To take a lead on the reconciliation of medicines following hospital discharge, working with secondary care colleagues, practice colleagues, patients and community pharmacists to ensure patients receive the correct medicines post-discharge. Provide clinical expertise in medicines advice and addressing both the public and social care needs of patient

    6. Support colleagues and patients with medication queries and drug monitoring especially with high risk drugs.

    7. Take responsibility for the medicines management of some patients with complex chronic diseases.

    8. Undertake clinical medication reviews to proactively manage people with complex polypharmacy.

    9. To work with patients who need support reducing long term prescribed Opiates and Gabapentinoids.

    10. To help tackle local inequalities and review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit, individual patient reviews and action planning.

    11. Contribute pharmaceutical advice for the development and implementation of new services that have medicinal aspects ( advice on treatment pathways and patient information leaflets).

    12. Contribute to the General Practice Quality and Outcomes Framework (QOF) and enhanced services.

    13. Work with general practice teams to ensure that practices are compliant with CQC standards where medicines are involved.

    14. Take a lead role in supporting further integration of General Practice with other healthcare teams to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.

    15. Take a central role in the clinical aspects of shared care protocols and liaison with specialist, community and anticoagulation pharmacists.

  • Undergraduate degree in Pharmacy
  • Registration with the GPhC
