We are recruiting a Care Co-Ordinator to support people living with a palliative care or end of life diagnosis across Birmingham to maximise their quality of life at home.
The Care Coordinator will work as part of a small team and be responsible for taking referrals, visiting clients, assessing needs, effectively organising and coordinating services/support, navigating clients to other appropriate support/services, advocating on their behalf or accompanying them as required, to support their end of life/palliative care journey.

Main duties and responsibilities:
- To work with clients and families enabling them to access appropriate support services to maximise their quality of life at home.
- To assess clients' needs using the Bridges assessment procedures.
- Visiting and meeting clients in their homes, hospital, hospice or other appropriate setting as preferred by client.
- To utilise excellent communication skills, with clients and other professionals including main referrer and key worker, providing timely feedback.
- To ensure tasks are completed effectively so that quality of support is maintained.
- When required support clients in accompanying and advocating on their behalf with different service providers and agencies.
- Provide clients with accurate information, identifying any other information needs and provide or navigate as appropriate. To record accurate data for service monitoring and evaluation including recording accurate up to date comprehensive notes on client files, working within the client information handling guidelines.
- To receive and process referrals coming into the service and to prioritise and action these accordingly.
- Record new referrals and client interventions onto existing database and maintain both the database and other records accurately.
- To provide regular reports as required, including end of quarter and end of year reporting.
- To support carers and dependents with bereavement and assist in accessing appropriate support
- Must have a full, clean driving licence and be able to travel in Birmingham and surrounding area, as well as travelling to Sandwell office.
