Job summary
TheSandwell Community Dementia Support Service (SCDSS) is commissioned by SandwellMetropolitan Council and the Black Country and West Birmingham ClinicalCommissioning Group. As lead provider, we are working alongside seven otherlocal organisations; Agewell CIC, Dementia Pathfinders CIC, West BromwichAfrican Caribbean Resource Centre, St Albans Community Association, BUDS, AgeUK Sandwell and Alzheimers Society to provide this service across SandwellMetropolitan Borough.
Main duties of the job
Supporting people with dementia and their carers inidentifying their needs, maintaining their independence, empowering them to make decisions and enabling them to remain in control of their lives. Triaging referrals, carrying out initialconsultations and in partnership with primary care, utilise appropriatescreening tools and co-develop care plans.

Job descriptionJob responsibilities
Working across Sandwell (Tipton,Rowley Regis, Oldbury, Wednesbury, West Bromwich and Smethwick) andneighbouring boroughs including Birmingham, Dudley, Wolverhampton and Walsall,you will ensure that you support people affected by dementia or people whomight be experiencing memory loss with the right and timely support that theyneed.
