We work collaboratively with public, private and voluntary organisations, educational settings and other partners to design and deliver sustainable innovative solutions to improve the lives of children, families, young people and adults from early years to end of life care.


The day-to-day duties


The Sandwell Community Dementia Support Service (SCDSS) is commissioned by Sandwell Metropolitan Council and the Black Country and West Birmingham Clinical Commissioning Group.

As lead provider, we are working alongside seven other local organisations; Agewell CIC, Dementia Pathfinders CIC, West Bromwich African Caribbean Resource Centre, St Albans Community Association, BUDS, Age UK Sandwell and Alzheimer's Society to provide this service across Sandwell Metropolitan Borough.


Supporting people with dementia and their carer's in identifying their needs, maintaining their independence, empowering them to take make decisions and enabling them to remain in control of their lives.

Triaging referrals, carrying out initial consultations and in partnership with primary care, utilise appropriate screening tools and co-develop care plans.


We are looking for a team member with:

  • Dementia qualification Level
  • Demonstrable experience of working proactively in a busy environment and proactively supporting colleagues.
  • Experience of CRM systems and database management.
  • Knowledge and/or experience of dementia care and supp
  • Empathetic listening skills.
