    Job summary

    An opportunity has arisen for an Advanced Nurse Practitioner to join our friendly are a well organised practice in Rastrick, Brighouse, who strive to consistently provide high quality care to all our patients. Our growing list size currently stands at over 4800 patients. We offer excellent appointment access without telephone triage and our patient feedback is consistently positive.

    The practice is a member of Calderdale ICB, Pennine GP Alliance, and the Lower Valley Primary Care Network.

    The hours are 20 hrs p/w to include Monday9:00-5:30pm, Tuesday 1:30pm-5:30pm, Wednesday 9:00-5:30pm.

    The ANP will have the freedom andauthority to act and make autonomous decisions in the assessment, diagnosis andtreatment of patients.

    They are an opportunity for patients to avoid the need to be referred to aGP, thereby enabling them to receive timely care and negate unnecessary delayin receiving treatment.

    Main duties of the job

    a. Undertake the collection of pathologicalspecimens on occasion.

    b. Request pathology services as necessary

    c. Process and interpret pathology andother test results as required

    d. Maintain accurate clinical records inconjunction with extant legislation

    e. Ensure SNOMED CT codes are usedeffectively

    f. Maintain chronic disease register

    g. Chaperone patients where necessary

    h. Prioritise health issues and interveneappropriately

    i. Support the team in dealing withclinical emergencies

    j. Recognise, assess and refer patientspresenting with mental health needs

    an extended and supplementaryprescriber, adhering to extant guidance

    l. Support patients in the use of theirprescribed medicines or over the counter medicines (within own scope ofpractice), reviewing as required

    m. Contribute to practice targets (QOF,etc.), complying with local and regional guidance

    n. Liaise with external services/agenciesto ensure the patient is supported appropriately (vulnerable patients, etc.)

    o. Delegate clinical responsibilitiesappropriately (ensuring safe practice and that the task is within the scope ofpractice of the individual)

    Support the clinical team with allsafeguarding matters in accordance with local and national policies

    Understand practice and local policiesfor substance abuse and addictive behaviour, referring patients appropriately

    Deliver opportunistic health promotionwhere appropriate

    About us

    Practice team consists of 1 GP principal, 3 salariedGPs, 3 Practice Nurses, an efficient reception / administration team. The Practice is lead by 2 part time Practice Managers.

    Longroyde Surgery is a SystmOne Practice and also uses PATCHs to communicate with patients. We are a small, supportive team who all share the same vision regarding patient care and this is often reflected in patient feedback.

    Essential

  • Registered nurse with Nursing and Midwifery Council
  • Degree qualified Advanced Nurse Practitioner with relevant clinical skills.
  • Independent prescribing qualification
  • Meets NMC revalidation standards.
