Job summary
We currently have an exciting role within our Enhanced Care Home Team within RWT Primary Care Network as a Band 3 Healthy Ageing Co-Ordinator. As a member of the team you will be identifying and supporting patients living with mild to moderate frailty and undertaking assessments for this group of patients.
You will be expected to run your own clinics and assess patients within their own homes.
It is essential that you have a full UK driving license and have access to a car that can be used for work purposes as the post will involve travelling across the city to provide designated clinics or domiciliary visits.
Main duties of the job
The post holder will have responsibility for maintaining standards, identifying risks, and supporting the functioning of older adult services, ensuring high quality services for our patients.
They will identify and support patients who are indicated to be living with mild to moderate frailty.
