**There are multiple vacancies across Norfolk.

Please note that should we receive a high number of applications, we reserve the right to close the advert earlier than advertised.

    Main duties of the job

Across Norfolk Healthy Child Programme we are developing an exciting opportunity for NMC registered practitioners to work with children and families from the antenatal period up to 19 years of age.

If you have a current Band 5 NMC registration, you are excited by innovation, and are passionate about the public health agenda and improving health outcomes for children, young people and families, then this may be the career choice for you.

You will have the opportunity to work both virtually and in person, gaining experience of our universal offer and providing targeted support within the community.

Please note that this role requires you to be able to travel to visit various bases, other services, and families at home.

    About us

Rated 'Outstanding' by the Care Quality Commission, we are proud to provide high quality innovative services across most of the East of England that enable people to receive care closer to home and live healthier lives.

There's a good reason why our services are outstanding - and that's our amazing staff - who, for the seventh year running, rated us incredibly highly in the national staff survey.

If you share our passion for innovative and high-quality care delivery, then please submit your application and join us on our exciting journey as a leading-edge specialist community provider. All are welcome to apply and our promise to you is a culture which prioritises staff engagement and development.

    Job description

    Job responsibilities

  • To work as part of a multidisciplinary 0-19 team to identify the health needs of children and young people, their families and the community and contribute to developing a strategy to address individual identified needs as well as those needs within the community.
  • To be committed to maintaining an evidence-based work ethic through continual learning and professional development. Postholder will be encouraged to develop professionally including consideration of the SCPHN course..

Postholders should ensure that they are compliant with safeguarding mandatory training and that they are aware of local safeguarding policies and procedures and how to access these.
