Referral Management Administrator


LOCATION Aylesbury Medical Centre Surgery, but the post holder may be required to work at any of Nexus locations.


JOB PURPOSE:


  • To ensure that the flow of referrals and reports coming in and out of Nexus is maintained to the required standard.
  • To support Nexus to develop positive and effective working relationships and communication systems between Nexus and stakeholders.
  • To ensure patients are informed of any outcomes of their care as directed by the clinical team.

MAIN DUTIES RESPONSIBILITIES

  • To ensure that referrals received are processed in a timely manner according to the training given and within the appropriate time scale.
  • Provide support to patients, practice staff and hospital/community trusts regarding referrals.
  • To gain and utilise good knowledge of referral pathways.
  • Process requests for reports and/or copies of medical records.
  • To ensure that referrals received are processed in a timely manner and in accordance with the designated pathways, practice policies and processes within the appropriate timescales.
  • To ensure that the information provided in the referral letter is accurately entered into the clinical system, with all attachments saved correctly.
  • To ensure that all processed referrals are sent to the destination specified by the clinician using approved methods
    ; either via the NHS ereferral service, Choose and Book orUsing NHSmail.
  • Process all referrals following the strict guidelines, pathways and checklists provided.
  • Manage assigned eRS GP worklists to ensure that rejected referrals are dealt with in a timely manner.
  • Check referrals meet the criteria of the providers.
  • To achieve and maintain daily performance targets for processing referrals.
  • To ensure all referrals and patient contacts are accurately recorded on the Nexus clinical system.
  • To address any electronic issues that may need resolving to assist the hospital with the referral.
  • Ensure deadlines are met and take appropriate action to make sure referrals are completed within a given timescale. Escalate to your line manager where there are risks of nondelivery.
  • To assist with any patient queries and arrange referrals.
  • Assist patients with enquiries regarding their eReferral booking.
  • Deal proficiently and effectively with telephone calls, ensuring patients, Nexus team and key stakeholders are dealt with in a professional manner.
  • To prepare reports for insurance providers and solicitors adhering to the current Nexus policy.
  • To provide copies of medical records adhering to the current Nexus policy.
  • Work closely with Cluster Administrators in relation to medical reports and copies of medical records.
  • Handle calls in respect of eReferral. Be able to understand and explain the eReferral process and be able to identify the patient referral using the appropriate system.
  • To deal with queries associated with the role.
  • Undertake other administrative tasks within the role if required.

The post-holder will:


  • Abide by the procedures and policies of Nexus Health Group
  • Maintain patient and data confidentiality
  • Work within the guidelines set by Nexus to ensure that the practice is safe, efficient and provides the best possible care for patients
  • Work in accordance with agreed standards of service delivery and within the team policies, procedures, and protocols
  • Ensure correct governance processes are adhered to in the handling and management of patient records and use of the electronic patient appointment system.
  • Maintain integrity in compliance with patient confidentiality and the Data Protection Act 201
  • Comply with the policies and guidelines around medical records
  • Ensure accurate read codes are used when entering data into the patients electronic records held within the Nexus clinical system.
  • Be proficient with a variety of electronic record systems required to undertake this role
  • Appropriately use practice equipment including IT software.
  • Use initiative and good judgement at all times.
  • Work effectively with the rest of the team in delivering the full scope of the service.
  • Work professionally within Nexus Health Group and liaise competently at all times to ensure the best possible patient experience.

  • Work closely with the IT team to ensure all systems are operational.

  • Participate in Nexus Appraisal process to identify and discuss professional needs.
