Senior Clinical Case Manager (Childrens)

Eden Brown Synergy are actively recruiting for an experienced Clinical Case Manager for an amazing opportunity we have available in the South East London area.

Our client are looking for someone who has significant experience in the field of NHS Children's and Young People's Continuing Care, commissioning for children and young people as well as having a good understanding of the national and local legislationto provide clinical and operational leadership within the Children's Complex Needs service and specifical in respect of Children's CHC.

To be successful in this role, you will need to:

Be able to provide and receive highly complex, sensitive or contentious information, negotiate with senior stakeholders on difficult and controversial issues, and present complex and sensitive information to large and influential groups.
Have the ability to analyse complex facts and situations and develop a range of options.
Have working knowledge of Microsoft office with intermediate key board skills.
Have a professional calm and efficient manner.

Key Responsibilities:

Leading a team of Nurses in the assessment, review, case management and commissioning of services in respect of Children's Continuing Care.
Supporting the business in driving transformation as well as value for money in planning and commissioning services in relation to Children's Continuing Care, Personal Health Budgets and transition to adult complex needs services, within the Surrey ICB'sChildren's Complex Needs Team.
Represents the ICB in relation to the commissioning of services for children and young people with complex health needs.
Work closely with Local Authority colleagues in Social Care and Education to ensure integrated commissioning for children and young people with complex needs.
