About The Service

The Independent Living Service is a multidisciplinary team which supports adults to remain independent at home and connected to their community, enabling people to take control of their own lives.

Independent Living Service also works closely with (Adult) Social Work colleagues and the Integrated Health and Social Care team to support people to live their lives well.


About the job

We are embarking on a third year of an ambitious Programme of change that involves transforming the way we work, our systems and digital technology, our business processes and procedures and culture by empowering colleagues and keeping the voice of the citizen at the heart of everything we do.

Due to the previous postholders internal career progression, we are seeking a new Outcome Delivery Officer to join our happy and hard working team.

The Outcome Delivery Officer will work collaboratively within our Programme team to support Senior Responsible Officers (SROs) through facilitating discussions about performance measures and financial reporting of the wide ranging portfolio of short term grants/projects across our Older Person Services.

The role is also responsible for reporting on behalf of the Local Authority to our partners within the Integrated Health and Social Care partnership timely and accurately until 31st March 2025.

You will have an active role in fostering and maintaining positive working partnerships within the Local Authority and across the Integrated Health and Social Care partnership, whilst being aware and sensitive to operational pressures and priorities.
