    Job summary

    Ash Trees Surgery is a 29,000 patient general practiceon the Lancashire, Cumbria border.

    Your role would be to work in the medicine management team within the GP practice to provide expert knowledge and support to the GP's and the wider practice team. Providing excellent care for patients, ensuring the practice follows local and national guidance in regards to medicine management.

    The practice is a training practice and have close links with local Universities. We will provide in house training and fully support ongoing professional development.

    Main duties of the job

  • Provide expertise in clinical medicine reviews and address public health and social needs of patients.
  • Reduce inappropriate poly-pharmacy and wasteful prescribing through clinical medications reviews.
  • Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive they need post discharge.
  • Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get best outcomes form their medicines and identifying and addressing medicine related issues.
  • Interface with community and hospital pharmacy colleagues, developing referral processes and promotion of repeat dispense services.
  • Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.
  • Manage patients and health care professionals medicine queries.
  • Act as a source of medicines information for all the practice team and patients, inclusing teaching sessions for the practice team.

    Ash Trees Surgery covers thesemi-rural area of the North Lancashire and South Cumbria border across 6sites.

    We work in a beautiful part ofthe country with the Lake District and Yorkshire Dales close by with goodtravel links via the M6 and West Coast rail line.

    Our clinical team includes 21GPs, 2 advanced nurse practitioners, 1 Nurse practitioner, 2 clinical pharmacists, 1 paramedic, 2 frailty nurses, 8 practice nurses, 1 NA, 4 HCAs plus support fromthe allied health professionals who are part of the PCN team.

    The administrative staff provide additional support to all roles.

    As a practice we encourage self development and support is given for courses and professional development including mentoring and supervision sessions.

    Job responsibilities

  • work as part of the medicine managements team within a GP surgery.
  • Provide expert knowledge in medicines ( including doses, side effects, adverse events, possible alternatives) and ensuring national and local guidance is followed.
  • Reduce polypharmacy and wasteful prescribing through medication reviews.
  • Contribute to reductions in medicine related hospital admissions by supporting patients to get the best outcomes form the medication and identify medication related issues.
  • Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicine review dates.
  • Increase quality and safety of prescribing through mechanisms such as audit and PDSA cycles
  • Manage practice formularies to improve quality , safety and cost effectiveness.
  • Work with primary care professionals and patients to implement NICE and other evidence based guidance.
  • Contribute to multimorbidity reviews and multidisciplinary reviews.
  • Review pathology results for patients on known medicines.

    Experience

    Essential

  • 2 year post graduate experience.
  • Desirable

  • experience of working in or of primary care.
  • Qualifications

    Essential

  • Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council.
  • Minimum of 2 years post graduate experience in pharmacy, as demonstrated within a practice portfolio.
  • Hold or are working towards a prescribing qualification.
  • Desirable

  • Experience of General practice.
  • knowledge of acute and long term conditions that are likely to be seen in general practice.
