    Job summary

    Wehave an exciting opportunity for a Clinical Specialist Physiotherapisttojoin our Integrated Community Stroke Rehabilitation TeaminBarnsley.

    Therole with encompasses intervention across the whole stroke pathways; SpecialistStroke Inpatient Rehabilitation Facility & Stroke ESD & longer-termStroke Rehabilitation for up to 6 months.

    As Clinical Specialist Physiotherapist,youwill act asan advanced clinical specialist responsible for the assessment & treatmentof patients, leading on complex cases, enhancing the effective delivery ofStroke NICE guidance.

    You will be part of the leadership team, promoting goodclinical practice, identifying & addressing any training needs &working with professional leads to support the professions development withinthe trust.

    Main duties of the job

    As Clinical Specialist you will:

    Act as an autonomous practitioner to undertake a significant specialist clinical case load, taking a leading role in the advanced assessment, clinical diagnosis & treatment of patients.

    Working with the service manager & clinical lead in planning, co-ordinating, delivering & evaluating the service

  • Participate in clinical governance including active involvement in audit & research.
  • To take a lead role in supervision & teaching
  • Contribute to the on-going development of clinical practice, standards, policies, protocols & guidelines.

    Have a key role in training & education, participate in regular supervision & appraisals contributing to clinical leadership facilitating a culture of learning/reflective practice.

    Have high level communication/interpersonal skills.

    Have a full UK driving licence & access to a vehicle.

    Job responsibilities

    JOB SUMMARY

    The Advanced Nurse/AHP Practitioner will be a member of the Stroke Services team and be responsible for the delivery of safe and effective nursing/therapy practice across the Stroke pathway, incorparating inpatient and community Advanced Nurse/AHP Practitioner will ensure clinical systems are in place to support and monitor practice and performance and support the service manager in the operational delivery of the service.

    The Advanced Nurse/AHP Practitioner will hold their own clinical caseload.

    Working with other lead Nurses / therapists the post holder will provide trust wide leadership and support within their own discipline.

    KEY RESULT AREAS:

    Clinical:

    Ability to: deliver a comprehensive and effective service topatients across the stroke pathway.. This will be achieved by:

  • Leading in the delivery of the nursing/therapy service within the team.
  • Providing specialist nursing/therapy assessment and intervention within individuals professional expertise to an allocated caseload of more complex cases held by the team.
  • Utilising specialist and advanced knowledge of therapy models of practice to guide intervention and assessment methods appropriate to individual patient needs.
  • Working with patients,families and carers to identify goals as part of the overall care plan, using assessment tools based on current evidence based best practice.
  • Supporting the culture and implementation of service user led care plans to achieve therapeutic goals.
  • Ensuring the nursing/therapy team monitors, evaluates and modifies treatment in order to measure progress and ensure effectiveness of intervention.
  • Showing a working knowledge of current legislation , safeguarding adults/Vulnerable Adult, Mental Capacity Act, Mental Health Act 1983 etc.
  • Ensuring that the nursing/therapy team keep abreast of new approaches and treatment developments, ensuring that staff establish and maintain effective documentation processes in line with Trust and Professional standards regarding Clinical Record Keeping.
  • Speciality Clinical Responsibilities

  • Providing expertise, leadership and supervision to Nurses/Therapists and support staff workingacross the stroke pathway.
  • Utilising resources effectively to resolve complex clinical and non clinical situations.
  • Broadening clinical research and development skills by leading on the evaluation of the service via service evaluation, audit and research projects to promote evidence based practice.
  • Promoting reflective practice amongst the team in light of new professional guidance and best practice.
  • Working with the Nursing/Therapy Leads to incorporate emerging research into positive clinical outcomes for patients
  • To liaise with other health professionals and local partnership organisations, to deliver interventions for patients across the pathway.

  • Ensuring all staff within the nursing/therapy service work as a team across the pathway

    Qualifications

    Essential

  • RGN or Registered Allied Health Professional.
  • Relevant Teaching Qualification or willing to undertake within an agreed timeframe.
  • Postgraduate study to Masters Level or equivalent in a field related to Nursing/Therapy.
  • Current NMC / HCPC Registration.
