    Job overview

    We are looking for an enthusiastic and dynamic Doctor to work at the Senior Clinical Fellow level at the Bedford Hospital Site in the Medicine division. You must be dedicated to providing high quality clinical care, maintaining clinical standards, and continually looking to drive positive change by developing the service.

    Posts are available for 12 months initially, shorter term appointments will be considered. These posts are aimed at doctors who would like to gain more experience in medical specialties before committing to a particular career.

    Your job will include a combination of direct clinical care, clinical education and personal development activities designed to support your career progression alongside delivery of the department's service objectives.

    Rotations through a variety of medical specialities which may include Cardiology, Stroke medicine, Elderly Medicine, Acute Medicine, Respiratory Medicine, Diabetes & Endocrinology and Gastroenterology. You will be assigned a clinical supervisor and will be encouraged to keep a portfolio of your experience as well as participating in all departmental educational opportunities and attending outpatient clinics whenever possible.

    The posts include an out-of-hours commitment, in line with our established registrar rotas, which are fully compliant with the European Working Time Directive. Out-of-hours works includes both acute admissions and ward-cover.

    Main duties of the job

  • Responsible for the admission, clerking and day to day care of their patients on the General Medical wards, as assigned.
  • Participate and conduct ward rounds as required
  • Participate in the registrar on call rota.
  • Participate in audits and current research projects. These are available through individual Consultants.
  • Attend the active postgraduate programme with weekly lectures, radiology, medical and pathology meetings and medical unit presentations.
  • Participate in the teaching of Foundation Doctors at the bedside as well as to Medical Students.
  • Attend practical procedure training
  • Involvement in Quality improvement projects
  • To maintain a good working relationship with existing staff members in the hospital.
  • To perform duties in occasional emergencies and unforeseen circumstances at the request of the appropriate Consultant in conjunction, where practicable, with their colleagues both senior and junior.

    Detailed job description and main responsibilities

    Title: Senior Clinical Fellow (General Medicine)

    Employer: Bedfordshire Hospitals NHS Foundation Trust

    Base Hospital: All posts are regarded as being Trust wide, but the base Hospital will be Bedford Hospital and there will not be an expectation for the successful candidate to work cross site

    Duration: Fixed Term 12 Months

    Educational Approval: the post has neither Royal College nor Postgraduate approval for training. The post is educationally sound and well supported, with excellent opportunities for gaining clinical experience in the specialties covered. However as a locally employed Doctor, there will be opportunity to be allocated login details to the local eportolio for your entries

    The post holder is managerially accountable to the Clinical Director of the Medicine Division, Dr Rajeev Kumar.

    Learning Objectives/Specialist Interests

    The post comprises of approximately 100% of direct clinical time working in medical specialties on rotation.

    If required and on mutual agreement, it is possible (maximum of 20% i.e. approx. 2 half days per week) to agree noon clinical sessions for the development of specialty clinical interests or areas of expertise in support of clinical practice. This is once clear achievable objectives have been established.

    This is a non-training post but the Clinical Fellows will get wide clinical experience, supervised by consultant medical staff.

    All rotations are planned to give a variety of acute and specialty work. They consist of 3x4 month posts

    DUTIES OF POST

    Routine and emergency patients must be seen and admitted expeditiously.

    Ward rounds and visits to individual patients as well as discussion with their relatives may be necessary.

    Keeping medical records as per GMC Good Medical practice is utmost; these must be dated and initialed, together with any other related administrative duties including discharge and clinic letters.

    The patient's discharge summary paperwork is to be completed prior to the patients 'discharge. Clinic letters are dictated within clinic and typed by the secretaries.

    Supervision of the Foundation Year 1 doctors when performing clinical duties.

    Duties in occasional emergencies and unforeseen circumstances at the request of the appropriate Consultant(s), in consultation where possible with colleagues both senior and junior. Additional commitments arising under this paragraph are exceptional. In particular, the doctor will not be required to undertake work of this kind for prolonged periods or on a regular basis.

    Agree to read and follow any protocols laid down (either in writing or verbally) for the firm in which you are working.

    In case of any difficulties ensure that advice is sought from a senior colleague.

    Essential criteria

  • Full GMC Registration and licensed to practice
  • MBBS, ChB or equivalent Part I MRCP or Equivalent
  • Part II Written or PACES or equivalent
  • Experience

    Essential criteria

  • Guidance Score 2 years in General Medicine at SHO level with experience of acute on call
  • Experience of managing Acute Medical Take and Acute Medical Emergencies
  • At least two years post-registration
  • Ability to assess, diagnose and treat acutely sick patients
  • Demonstrable qualities in clinical procedures, including: CVP Insertion, Chest Drain insertion, Lumbar Puncture, Advanced Life Support
