Description
An exciting opportunity has arisen to join the Combined Community and Acute Directorate. We are seeking an enthusiastic individual to join the Therapeutics and Palliative Care Administration Team as an MDT Co-Ordinator.


You will play an integral part in the overall development and delivery of an effective Palliative Care MDT and will provide support to the A&C Team Leader, deputising accordingly and providing supervision to staff members within the department.


The role will be a point of contact for Palliative Care consultants and specialist nurses and will work closely with them to prepare for and attend MDT meetings, capture outcomes, update the relevant information systems with accurate and timely information, and ensure that actions from MDT meetings are completed.

Identify patients for discussion at MDT meetings.

Organise MDT meetings, including preparation of a suitable location or virtual meeting environment.

Attend weekly MDT meetings and capture discussions/outcomes.

Ensure clinical decisions taken at MDT meetings are communicated to relevant professionals.

Update relevant trust/patient systems.

Continually develop, improve and refine the MDT process.

Excellent communication and interpersonal skills.

Be proficient in the use of all appropriate electronic systems and standard operating procedures in relation to patient data.

Use own judgement and initiative to resolve straightforward enquiries and issues courteously and efficiently.
