Description

Obtain referrals electronically and in person from third party sources as Hospitals, Mental Health Liaison Teams and GPs. Liaise with hospitals as part of discharge planning to effectively support within the community.

Conduct Triage and assessments for all referrals from Hospitals and GP, as well as assessing referrals from GP surgeries and any self-referrals where mental or physical health barriers would impact on access to treatment.

Utilise home visits and community venues for one to one and group work sessions.

Make referrals to statutory and community organisation to address physical and mental health barriers. Ensure a MDT approach to caseload management to ensure lead agencies are involved and actively supporting service users. Any other duties as agreed with management in line with the remit of the role.
