    To be actively involved in providing Advanced Clinical Skills, theoretical knowledge, pharmacy medicines management service, and practical skills to Mersey Care NHS Foundation Trust. The post holder will act as the advanced clinical pharmacist and Non-Medical Prescriber (NMP) for the Later Life and Memory Services (LLAMS) community mental health within the medicines management team. The post holder will ensure that they deliver an advanced level of clinical practice and provide medicines optimisation characterised by a high level of autonomy and complex decision- making. They will be responsible for leading, delivering and developing comprehensive clinical practitioner and pharmacy services to the Trusts LLAMS, providing specialist information and advice to all Members of the Multi-Disciplinary Team (MDT) to maximise patient care. The post holder will be responsible for training and supervising other LLAMS and Medicines Management staff members within this specialist area of dementia.

    Main duties of the job

  • Advanced theoretical and practical knowledge of a range of procedures relating to dementia, and practices, including differentia diagnosis.
  • To work autonomously as an Advanced Clinical Pharmacist/Practitioner to provide effective care to individuals and groups, managing a caseload of patients within LLAMS delivering individualised direct patient care.
  • Demonstrate a critical understanding of their broader level of responsibility and autonomy and limits of their own competence and professional scope of practice.
  • To act as the Trust's point of contact for pharmaceutical issues within community mental health services of the LLAMS Team.
  • To be responsible for leading, and delivering an advanced level of clinical expertise and pharmaceutical care, within the LLAMS, to undertake advanced health assessment, from patients initial presentation, clinical assessment, requesting and interpreting of necessary diagnostic clinical investigations, interpretation of information leading to diagnosis of patients with dementia, and formulating treatment and management plans on behalf of the Trust, all of which require interpretation, judgement, and comparisons involving highly complex facts or situations, in analysis.

    Detailed job description and main responsibilities

  • To ensure that effective clinical and Medicines Management practices are developed and carried out consistently within LLAMS community mental health services within the Trust, to promote advanced clinical practice, medicines safety and reduce risk of harm from both mental health and physical health medications.   • To develop evidence-based treatment protocols for use within the LLAMS community mental health and the Trusts older adult inpatient wards. To facilitate the implementation of local pathways and national guidelines relating to dementia for the Trust, and External stakeholders such as Universities.
  • To Lead if necessary and contribute to the development of new policies and procedures relating to dementia and the LLAMS, the modification of existing policies and procedures, and their implementation.
  • To undertake clinics with direct patient contact face to face e.g., house visit, clinic, Digital e.g., Teams, around anywhere and Verbal e.g., telephone, including clinical assessment, requesting and interpreting clinical information, assimilating diagnosis, delivery of diagnosis, leading on treatment plans, including mental health and physical medication reviews, providing an expert response to fitness to drive requests and post diagnostic follow up considering social, and psychological follow ups in providing high level of patient and Carer/Family care.
  • The post holder can seek clinical supervision or advice from the Consultant Psychiatrist when needed for any complex or high-risk cases.
  • To actively initiate and take part in practice research relating to specialist area of practice, which may lead to publication or to the presentation of papers or posters at conferences and workshops

    Qualifications

    Essential criteria

  • Master of Pharmacy degree
  • Postgraduate diploma in Clinical Pharmacy, or equivalent experience
  • Experienced Independent prescriber qualification
  • Specialist Practitioner in the Assessment and Diagnosis of Dementia, or willingness to work towards
  • Registration with General Pharmaceutical Council
  • Postgraduate qualification in Psychiatric Therapeutics/Clinical Pharmacy
  • Desirable criteria

  • Membership of the College of Mental Health Pharmacy (CMHP)
  • Membership of the Royal Pharmaceutical Society (RPS)
  • Leadership or management qualification/training
  • Knowledge/Experience

  • Experience of working within an NHS organisation and knowledge of the national and local NHS healthcare agenda and its implications
  • Substantial autonomous experience working as an Advanced Clinical Pharmacist/Practitioner in a multi professional practice setting as part of a multidisciplinary team
  • Experience in developing and implementing governance safety frameworks to support service and quality delivery, improvement and sustainability including learning from incidents or quality assurance audits/ patient and carer feedback and National policy
