    About the Role Ambrose Avenue Group Practicehas a fantastic opportunity for an advanced nurse practitioner to join our team and be part of a team that makes a difference. You will be supported by a diverse, multi skilled, professional team of clinical and non clinical staff providing high quality, person centred, holistic nursing care. You will provide a first point of contact for patients presenting with undifferentiated, undiagnosed problems, using skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis management plan. You will work collaboratively with the multi-disciplinary practice team to meet the patient's needs. About the Candidate All applicants should have the following qualifications, registrations, skills and experience:
  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Qualified Triage Nurse
  • Minor illness qualification
  • Postgraduate Diploma (Advanced Practice qualification).
  • Experience working in a General Practice
  • Experience working within a nursing team
  • Experience in dealing with vulnerable patients
  • Proven ability to evaluate the safety and effectiveness of their clinical practice,
  • Experience in nurse-led triage
  • Experience in prescribing and undertaking medication reviews
