At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier It's a super exciting time to join us as we begin to deliver the second iteration of our Integrated Health and Wellbeing Service (IHWS) throughout the County of East Sussex, and we're looking for a full time Weight Management Practitioner to join the team


Our IHWS programme is designed to support individuals to improve their health behaviours and make sustainable changes to their lifestyle.

The programme consists of key services such as;

  • Stop smoking support
  • Tailored physical activity programmes
  • Adult weight management _(this is where you come in)_
  • Health Coaching
  • NHS Health Checks


Residents across the East Sussex will be able to benefit from a variety of our established pathways and products, including Gloji Groups, Gloji digital and Man V Fat football.

Gloji Groups is a 12 week weight loss programme, allowing our clients to benefit from group support and encouragement each week, covering topics such as making and breaking habits, nutrition, sleep and getting active

A typical day as a
Weight Management Practitioner might include:

  • Delivering evidence-based interventions covering nutrition, movement, mind, sleep and alcohol, with a focus on behaviour change and psychological support
  • The delivery of a small amount of remote provision (done usually via teams)
  • Motivating, inspiring and supporting individuals to lose weight and adopt a healthier lifestyle, through a professional, knowledgeable and empathetic approach
  • Implementing aspects of an individual's health improvement plan including SMART goal setting, taking into account their risk factors, individual lifestyles and motivations
  • Ensuring an excellent and safe experience for clients, aligning with our internal policies and procedures
  • Supporting our new "Move More" pathway which will help support local residents get back into local sporting opportunities (e.g Swimming).
  • Supporting referral generation activities to promote the service including outreach and primary/secondary care liaison
  • The role will include some evening work to cater for our client demands. This will be AT LEAST 2 evenings per week (this may increase on the odd week when "cover" across the county is required)
  • Accurately recording and monitoring data, including client progression
  • Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments we're always looking to share knowledge and resources
  • Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance
  • Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values

To work with us in this role you will need to have some experience of supporting individuals to make healthy lifestyle choices, preferably in group settings, through nutrition, physical activity or health coaching.

You will feel confident managing a caseload and have knowledge of behaviour change techniques. A relevant qualification (such as health coaching or fitness instructing) would be a bonus
