    Job summary

    The Ashton Primary Care Network are looking to employ a full time Pharmacist to work alongside a team of PCN Pharmacists across the 8 Practices in Ashton-Under-Lyne. Part-time hours will also be considered. You will be working alongside a growing team consisting of General Practitioners, ANPs, Practice Nurses, Physician Associates, Paramedics and HCAs. This team based care approach ensures that patients are better cared for in the community.

    ClinicalPharmacy in General Practice is part of an exciting programme of transformationto develop a new model of care across the Primary Care Network. Overarching goals are to help keep patients safe andwell out of hospital and reduce the demands on general practice regardsmedicine use.

    Main duties of the job

    We arelooking for a Full-time Pharmacy Technician to join our existingGeneral Practice Clinical Pharmacy Team. We are looking for a dynamic and pro-active Pharmacy Technician to deliver and develop medicines optimisation services, improvepatient outcomes and support the primary care workforce in general practice

    Main dutiesof the job

    To maximisesafe, cost-effective, prescribing to improve the quality of patient care.

    Support andhelp patients to get the best from their medicines,

    Useexisting protocols to maximise cost-effective preparations,

    Improverepeat prescribing processes (including use of electronic repeat dispensing andonline ordering),

    Minimisingclinical risk and reducing avoidable wasted medicines.

    Develop andencourage a positive culture of medicines optimisation principles to assist thePCN in achieving local and national medicines-related objectives.

    Ashton Primary Care Network consists of 8 practices in Ashton-Under-Lyne who work collaboratively with the aim to provide holistic and coordinated care for patients. We cover a population size of approximately 62,000 patients.
