    Job summary

    Thesuccessful candidate will start with us inAugust 2024. This post is to work within the Emergency General Surgical (EGS)service. It is a Monday to Friday only post with daily hours of 8am until There is no on call commitment. The successful candidate will haveundertaken at least 4 months of general surgery in their foundation programme.

    Fulltravelling expenses are paid from a UK port of entry, and the appointee may beable to take advantage of Jersey taxation policies.     Main duties of the job

    Thispost is to work within the Emergency General Surgical (EGS) service. It is aMonday to Friday only post with daily hours of 8am until There is no oncall commitment. The successful candidate will have undertaken at least 4months of general surgery in their foundation programme.

    TheEGS service is led by consultant surgeons Miss Rebecca Fallaize (EGS Lead) andMr Muhammad Aleem (Acting Consultant). You will be responsible for helping withthe EGS in-patients, assisting in theatre for EGS cases and helping with theadmission of new GP and ED referrals to the service alongside an on call F1.The breadth of patients seen through the EGS service encompass GI, breast,vascular, trauma and urology specialties.

    TheEGS service is an evolving service which is presently undergoing redesign. Thesuccessful candidate will be actively involved in service redesign work withsubsequent ability to present project outcomes locally as well as potentialsubmission to professional meetings. Thesuccessful candidates will be able to combine service commitment with significantprofessional development in emergency general surgery. This is an ideal post togain experience towards applying for core surgical training or to add to coresurgical training prior to application for higher surgical training . To support the post holders continuous professional development andcareer aspirations Clinical Fellows will be supported by Consultant Clinicalsupervisors.

    Job description

    Job responsibilities

    Thispost holder will work full time within the Emergency General Surgery (EGS)service. On a day to day basis this involves being part of the daily inpatientward round, supporting the F1 with any in-patient issues, helping with the EGSadmissions and referrals from both the Emergency Department and local GPs, andassisting the EGS consultant in theatre.

    Intime there is expectation that the EGS service will run daily Same DayEmergency Care (SDEC) clinics which the post holder will be expected to helpwith, gaining autonomy in EGS care. There is a plan to introduce hotgallbladder and expedited surgical lists which the post holder will be firstassistant for. Depending upon the experience and needs of the post holder, it isexpected that they will learn to undertake abscess drainage, insertion ofsuprapubic catheter, laparoscopic appendicectomy and other appropriate surgicalprocedures during their time in post. It may be possible to attend minor opsand dermatology lists as long as the EGS work takes priority.

    Wards

    Thegeneral surgical ward team is comprised of 5 F1s and an F2 doctor. Due to oncall and leave commitments there are usually 2-3 F1/F2 doctors on the ward eachday. One F1/F2 will be present on the EGS ward round and be primarilyresponsible for the ward jobs although it is expected that the EGS CF (thispost) will support the management of the EGS inpatients as needed, undertakingward administration and advising on management as required. The post holder isalso expected to support the other foundation doctors on the ward as necessaryand to cover the general surgical inpatients during protected foundation doctorteaching.

    Flight transfer

    Jersey is aligned to the UKhealth system and relies on certain centres for tertiary care. Flight transfersare provided by a hospital based Jersey Emergency Transfer team (JETs).

    Patients being transferredacutely to tertiary centres such as the Cardiothoracic Unit in Oxford and tothe Neurosurgical Unit in Southampton for further specialist treatment areaccompanied by a doctor and a trained nurse.

    Appointed Clinical Fellows will beexpected to be trained to carry out flight transfers. Participation intransfers that take place out of hours will be on a voluntary basis and willcommand additional remuneration.

    Additional Professional Development

    Medical students fromSouthampton University are attached for 8 week rotations in their final year.Clinical Fellow post holders have enjoyed interaction with these enthusiastictrainees and have provided bedside teaching in addition to that provided byConsultants and Middle Grade Doctors. Clinical Fellows have organised mock OSCEin the 7th and 8th weeks of their attachment inpreparation for their final examinations. This has been a mutually beneficialexercise.

    The Education Centre is a sharedfacility for Health & Community Services Department and is equipped with anewly built Clinical Skills Lab which has assisted Clinical Fellows in selflearning, particularly in procedural work and optimising theircardio-respiratory resuscitation skills, and in teaching their more juniorcolleagues both medical students and nursing staff.

    There are designated clinicalfellow tutors and teaching programme which is in protected time which the postholder will be expected to attend.

    The duties of the post holderare expected to be flexibly worked, as inevitably a number of activities runconcurrently.
