This is an exciting opportunity for an enthusiastic individual interested in Performance management and improvement in CNWL Community Mental Health Service.

The post-holder will need strong data analysis skills, innate problem-solving ability and excellent communication skills. You should have experience analysing large quantities of data, developing new information and reporting solutions.

You may also be required to carry out some interventions when required, with a plethora of training and development opportunities.


The post-holder will be a self-motivated individual who has an organised and logical approach to work and enjoys working flexibly under pressure in a busy team and working on your initiative to achieve deadlines.

The successful applicant may have contact with patients or service users.

The post holder will work with the Brent Community SMT to developed a plan to imbed and monitor the provision of Step 2 interventions, and act as a specialist within the CMHT for Brent.

The post will sit within the Community management team, and will work with adults CMHT teams' managers to ensure robust implementation of step 2 across the teams.

The post holder will provide specialist support in the development and implementation of Step 2 interventions.

This will involve undertaking clinical work and interventions with clients and indirect advice guidance and monitoring of interventions provided across the CMHT.

Interventions will include working with the multi-disciplinary team.

The post holder will work autonomously and offer specialist support, and intervention plans for clients accessing step 2 interventions within Brent.

Brent is a place of contrast and a destination for thousands of British and international visitors every year.

Home of the iconic Wembley Stadium, the world-famous Wembley Arena, the new London Designer Outlet and the spectacular Swaminarayan Hindu Temple, it provides a stage for cultural, sporting and religious events of national and international importance.

The team serves the diverse and vibrant population of Brent, named London Borough of Culture 2020 and most recently 'Council of the Year' at the LGC Awards 2020.

The team base is located in Kilburn with excellent transport links.


The role will play a lead role in the development of effective step 2 transformation model to support a high-performance culture.


This will involve but not be limited to:

  • Monitoring the Step 2 waiting list and working with CMHT operational managers to ensure timely allocation and that a robust governance process is in place to monitor this.
  • Ensuring that all staff within the CMHT have the competency and skills to delivery step 2 interventions both group and 1
  • Delivering training to staff on the delivery of step 2 interventions within the CMHT as appropriate
  • To provided interventions, modelling approaches and training others in the team
  • To support the service manager, and operational managers to provide effective leadership and management of cases in a timely manner.
  • Promote and monitor safe and effective practice across the community pathways.
  • Enhance the patient/client experience ensuring effective engagement with services users and carers and the use of dialogue plus care planning
  • To work as part of a multidisciplinary team approach and contribute to the initial and ongoing assessments of clients referred to Brent CMHT and ensuring that step 2 is imbedded into the MDT process.
  • Ensure national standards and local guidelines, policies and care pathways are effectively delivered
  • To develop and implement the use of digital technology to enhance the monitoring, support and care provided to care home patients and carers
