    The Role:

    We are seeking a skilled, enthusiastic, and self-motivated Reablement Practitioner, to join our Adults' Health and Care Reablement Service in the Mid of Hampshire, based in Winchester.

    As a Reablement Practitioner, you will be responsible for carrying out holistic assessments of need and planning interventions to support across a multi-disciplinary team of Health and Social Care professionals, within the Reablement Service. This includes a focus on supporting hospital discharges and prevention of admission through the wider hospital system with internal and external partners. You will work alongside our Rapid Support Service (RSS) which is a short-term reablement and domiciliary care provider offering timely discharge of individuals who may have an eligible Care Act need. These individuals may need support with discharge from a hospital setting to their own home or may require support to prevent the need for a hospital admission, thereby supporting flow and effective management within the wider system. You will be multi skilled in understanding and completing the care management process and delivering therapy-based interventions with support from professionally qualified staff. There will also be an expectation to support the Reablement triage function.

    You will be part of a progressive team, using an enabling, adaptive approach, supporting people to live in their own homes where possible, and in the least restrictive way. You will be able to demonstrate effective assessment, planning skills, goal setting and work in a fast paced, evolving environment. You must be able to demonstrate strengths-based approach to provide advice and information to enable service users to problem solve their own needs and make informed decisions on their Reablement journey. You will have excellent communication, inter-personal and team player skills.

    Although there will be a primary focus on supporting the Rapid Support Service (RSS) there will be opportunities to work across Hampshire as part of a county wide Reablement Service comprising Occupational Therapy, Sensory Service and CQC Registered Community Response Staff. Previous experience in a Reablement, Rehabilitation, Health or Social Care setting is essential. Travel within Hampshire will be required, and you will need to respond flexibly to these requirements.

    This post will include 7 day working on a rotational basis, details of working pattern will be discussed at interview. We value our staff highly and offer full induction, supervision, and performance reviews, CPD opportunities and professional, clinical, managerial and peer support.
