Step Together - an exciting service for people with complex needs in Somerset.
We work with clients and partners creatively to develop new and sustainable support and adults facing difficulties in obtaining and sustaining housing and with mental health needs, drug and alcohol problems, behavioural issues, significant debt, and involvement in the criminal justice system.
The service offers a psychological, adversity and trauma (PAT) informed approach, advocates for system change, creatively thinking with other agencies to develop innovative ways to overcome barriers to engagement.
Working to the Recovery Coach Model principles, you will develop strengths-based actions orientated around the Outcomes Star.You will engage and support your own caseload of clients with complex needs in leading their journeys, and maximising their choice and control over the support they receive by developing a responsible and individual network of support.
You will have an understanding of issues relating to homelessness and mental health, and be able to build and sustain relationships with clients.
We work in a recovery focused, psychological, adversity and trauma (PAT) informed way in all aspects of the team's practice using relevant interventions, including Assessments, Support Planning, Risk Assessing, Brief Interventions and Solution Focused Therapy.
You will receive training, and on-going supervision and benefit from both operational and reflective supervision, case formulation and restorative peer support.
In addition to this you will be part of a close knit and supportive team of colleagues with opportunities for shared practice and collaborative working.
