    An exciting opportunity has arisen for an experienced Diabetes Specialist Nurse to join our existing team, providing Adult Diabetes Services across the Salford locality, to support Maternity Leave . Our Integrated service model encompasses a multidisciplinary approach to Diabetes Care, including partnership working with diabetes consultants, diabetes specialist dietitians and ANPs, allowing shared responsibility for the person with diabetes.

    The service aims to maximise the opportunity for preventing complications of diabetes through an effective empowerment and a collaborative approach, focusing on individual care planning, support for professional and patient education, emotional and psychological care.

    Main duties of the job The main responsibilities of the post:

    Working within both acute and community services the successful candidate would be expected to work within a team of skilled diabetes team members , delivering a high standard of evidence based clinical care to patients, and their carers working as part of the Integrated Diabetes Service. The successful candidate must be confident to deliver education sessions to both patients and healthcare professionals. The successful candidate will require excellent communication skills and will have the ability to work alone and within a multidisciplinary team. We are seeking a highly motivated individual who will contribute to the developments of our multi-disciplinary specialist diabetes service, and who has a commitment to continued personal development.     This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months.

    • Registered Nurse on Part 1 of the NMC
    • BSc Adult Nursing

    • Non Medical Prescribing
    Experience Essential
    • Evidence of experience working within the diabetes specialism
    • Knowledge of relevant strategies for diabetes and nursing care
    • Working as member of a multi-disciplinary team.
    • Delivering Group education to patients and health care professionals
    • Evidence of audit/service development
    • Ability to plan, organise and work under pressure
    Desirable
    • DAFNE Trained
    • Experience in diabetes related technology
    Knowledge Essential
    • An awareness of Diabetes Clinical Governance
    • knowledge of national diabetes care priorities
    • Up to date knowledge of all aspects of diabetes care
    • Is able to demonstrate an understanding and empathy relating to the condition of diabetes and the challenges facing the patient
    Desirable
    • Experience of working within both a diabetes acute and community setting
