About this role
This position functions as a case manager with the allocated caseload providing a range of therapeutic interventions, education, counselling and care of mental health consumers, their carers and family and care coordination with other services; undertakes mental health assessments, inclusive of risk assessments, mini mental state and any other assessments as deemed pertinent with consumers at the clinic and community settings; utilises a recovery oriented framework; initiates and participates in quality improvement activities in the development and review of performance indicators inclusive of National Outcome Measures; coordinates and administers the Sunflower Villas and Community Options accommodation program with the community mental health service including monitoring and reporting on key performance areas, policy and procedure development and strategic planning.
The MHPHDS Community Adult Mental Health Stream aims to provide evidence based, best practice, specialised mental health services supported by strategic planning in partnership with service providers and consumer and carer stakeholders.
The Community Adult Mental Health Stream is committed to working in partnership with its' communities to promote 'optimal mental health'. By treating consumers, carers and other services with respect and integrity whilst providing excellence in care, we foster a sense of hope for the consumer, whilst working as part of a team (mental health services and other services) providing leadership through case management to ensure holistic health care.
This role reports to the Professional Lead Occupational Therapy.
