In respect of trust wide Patient Safety Lead, lead the delivery and management of patient safety including: Incident reporting and management Serious incident processes including investigations Mortality surveillance Effective use of the Ulysses Risk Management System Effective implementation of the Duty of Candour Safe systems to prevent harm, including prevention of Never Events and managing risks in the clinical environment Inquest liaison Claims Management Risk Management In respect to Patient Safety Specialist, to provide expert support to the organisation, with direct access and routes of escalation to senior leaders, to raise patient safety issues or concerns including: Support transition from National Reporting and Learning System (NRLS) and Strategic Executive Information System (StEIS) to the new Learn from Patient Safety Events (LFPSE) service Involvement in implementing the new Patient Safety Incident Response Framework (PSIRF)

Improving safety culture Responding to National Patient Safety Alerts Implementation of the Framework for Involving Patients in Patient Safety Improving patient safety education and training Addressing patient safety improvement Implementing medical examiners In respect of all roles:

Provide timely, accurate reports as required relating to areas of responsibility for use from ward and team level to the board.

Ensure effective use of and analysis if qualitative and quantitative information to understand risk and support organisation learning Effective use of systems to support sustained organisation learning
