An exciting opportunity has arisen for a highly motivated and enthusiastic Band 4, Pathway Coordinator, within OpCourage The Veterans Mental Health and Wellbeing Service based at Vermont House, Washington.


OpCourage The Veterans Mental Health and Wellbeing Service is a collaboration between Integrated Care Board commissioned and funded mental and wellbeing health services, NHS England commissioned services including the bespoke veterans' health services and the non-statutory sector.

OpCourage builds on the existing statutory provision within local areas to ensure that a veteran can access a wider holistic service that brings together mental health, physical health and wellbeing care provision to meet the veteran's individual need.

OpCourage in the North of England are a wider provider collaborative of 7 partner organisations to deliver the new model including:
Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust

Tees, Esk & Wear Valley NHS Foundation Trust

Leeds and York Partnership NHS Foundation Trust

Pennine NHS Foundation Trust

Humber NHS Foundation Trust

Walking with the Wounded - 3rd Sector Veteran Organisation

Combat stress - 3rd Sector Veteran Organisation


This role will be based within Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust alongside the North East clinical team to ensure close liaison and collaboration with the other 6 providers for the entirety of the wider North of England OpCourage services.

The role is to provide coordination and support to the multi-disciplinary team in their management of patient pathway(s).

To ensure that referrals and clinical information is handed to the relevant partner within the collaborative in the North of England

To assist the clinical leadership team in the Veterans services with data flow and capture and prepare relevant updates to the wider clinical management team and NHS England

To capture and manage patient information on the relevant computer systems (ie RiO) to enable high quality and timely reporting

To monitor the flow of patients on assessment and/or treatment pathway(s), and escalate potential problems at the earliest opportunity

To act as an interface between patients/carers/professionals and lead clinicians.

The applicant will need a proactive innovative approach and have experience of developing and managing systems.


A high level of organisation and excellent communication skills, both written and verbal, along with extensive experience of the Microsoft suite and typing skills are required.

To be able to demonstrate a high degree of confidentiality and discretion.

You should currently hold the required level of qualifications, as outlined in the Essential Criteria of the job description.

Post is full-time - Monday to Friday - 9am to 5pm (times are negotiable)
