    Job summary

    We are looking for a Senior Childrens Continence Nurse tojoin our Surrey wide Childrens Integrated Continence Service, based atLeatherhead Clinic. If you are a registered nurse and would like theopportunity to work within a community setting within the field of childrenscontinence, this opportunity could be for you. This is a full-time permanent post, job shareand flexible working considered.

    You will work as part of an experienced, motivatedskill-mixed team providing evidence-based continence advice and support topromote continence to children ages 4-19 years.

    You will be responsible forproviding an holistic approach, including continence assessments, advice andsupport and reviewing progress for children and young people who present withdaytime enuresis, faecal incontinence, chronic constipation and impaction. Theservice is delivered in a variety of settings Surrey wide supporting childrenand young people including home visits, schools, specialschools and other community settings and telephone support. The role includesworking in partnership with health colleagues, education and other agencies topromote continence. We provide in house and external training to furtherdevelop your skills within the community setting and within the service tosupport the children and young peoples continence needs within Surrey.

    Main duties of the job

    The post holder will be responsible for overseeingthe provision of continence, assessment, management, advice and support forchildren and young people (CYP), aged 2-19 years, with identified continenceneeds daytime enuresis and constipation. A high proportion of these CYPwill also have severe/profound and multiple learning and physical difficulties.This will be achieved by providing expert training, advice and support to theCYP, their parents and carers, school staff and other staff members of Childrenand Family Health Surrey.

    Job description

    Job responsibilities

    The post holder will be responsible for overseeing the provision of continence, assessment, management, advice and support for children and young people (CYP), aged 2-19 years, with identified continence needs daytime enuresis and constipation. A high proportion of these CYP will also have severe/profound and multiple learning and physical difficulties. This will be achieved by providing expert training, advice and support to the CYP, their parents and carers, school staff and other staff members of Children and Family Health Surrey.

    Principal duties to include:

    Communication skills (factor 1)

    1. Through liaison with parents, carers, professionals and CYP, develop and maintain communication with people about difficult matters and/or in a difficult situation

    2. Responsible for communicating and receiving sensitive, complex, and sometimes contentious information relating to CYP, relatives, and carers whilst ensuring confidentiality. Use developed persuasive, motivational, negotiating skills, where agreement and cooperation is required, or where there may be barriers to understanding.

    4.Establish effective, appropriate communication processes with parents/guardians and carers.

    5. Communicate effectively with a wide range of health, education and social care professionals across professional boundaries. Be responsible for communicating information both within the organisation and outside it.

    6. Establish and maintain effective channels of communication within own team and wider CFHS team

    7. Participate in and contribute to meetings and working groups that inform service delivery, the organisation and partner agencies as appropriate.

    Analytical and judgemental skills (f3)

    1. Act within local and national Safeguarding (Child Protection) Policies and Procedures and take appropriate action as required.

    2. Responsible for ensuring the Lead for Childrens Continence Service and other members of the team, including relevant administrators, are up to date with all matters affecting the service provided to the CYP and their family.

    3.Accountable for the assessment and interpretation of information regarding CYPs continence needs and taking of appropriate action

    4. Able to use audit and research within the scope of the post.

    Planning and organisational skills (f4)

    1. Contribute to the planning, development and implementation of systems and procedures that ensure that the Childrens Continence Service delivers responsive care within the allocated resources.

    2. Ensure that own role and responsibilities are clearly defined and the clinical competencies required for these are identified and met.

    3. Responsible for providing cover for the Lead for Childrens Continence Service as required.

    4. Responsible for the planning and organisation of the Daytime Enuresis pathway of the Childrens Continence Service including prioritising client needs and service deliverables according to the demands of the service.

    Physical Skills

    1. Undertake such physical tasks as may be required to support the assessment and management of CYP in relation to their continence needs, which may include examination, catheterisation, application of appropriate containment products (following training and achievement of competency.)

    to different sites across Surrey and occasionally attend meetings/training etc outside of Surrey.

    4. Basic keyboard skills.

    Patient / client care (f6)

  • Undertake complex and non-complex continence assessments/reviews as required.
  • Plan, deliver, evaluate and adjust care as a result of these assessments in line with evidence-based practice and National guidance, in line with the requirements of the service and organisation.
  • Provide specialist clinical guidance to other members of CFHS, CSH Surrey and other agencies
  • Participate in the management of emergency situations including major incidents
  • If required, participate in child protection case conferences, network meetings, and other multi-agency meetings and support team members in the safeguarding process.
  • Participate in the delivery of a holistic, autonomous & clinical service. Make initial assessment, to plan & implement care, appropriate to the physical, psychological and social needs of the child & family. Empower families and schools to meet the CYPs needs in relation to continence and work in partnership with them.
  • Have sound practice knowledge developed through experience within the specialist practice area and have the ability to advise clients on individual treatment programmes and their potential outcomes.
  • Assess complex health and well-being needs and develop, monitor and review care plans to meet those needs in relation to continence.
  • Identify client-specific goals and interventions required to maximise health and wellbeing and develop effective approaches to meet the needs of CYP and their family.
  • Support the development of quality initiatives Essence of Care.
  • Evaluate the effectiveness of care and review care plans accordingly.
  • Respect individual wishes, beliefs and consent to treatment ensuring dignity is maintained at all times.
  • Determine and record treatment plans which are consistent with the outcome of anassessment, consistent with the CYPs/familys wishes and views in collaboration with other agencies and professionals.
  • Policy and service development (f7)

    1. Ensure the promotion of evidence-based practice by reviewing and monitoring standards of care in line with local and national guidance and research.

    2. Liaise with other practitioners within childrens services locally and nationally to communicate good practice and ideas with the aim of each CYP receiving the best possible care

    3. Act as an effective member of the multi-disciplinary team by attending meetings and contributing to the implementation of agreed standards of care.

    Financial and physical resources (f8)

    1. Responsible for overseeing the ordering of containment products needed for individual CYP.

    2. Responsible for ensuring the safe use of any specific continence equipment.

    3. Responsible for ensuring the annual servicing of any specific continence equipment.

    4. Responsible for ensuring and monitoring cleaning of any specific continence equipment.

    Human resources (f9)

    1. Provide cover and support to other members of the daytime enuresis team as directed by the clinical team leader

    2. Participate in clinical supervision to sustain and develop practice, utilising the service to manage all aspects of personal, professional and work related issues including distressing and stressful situations.

    3. Provide and support a teaching programme that supports delivery of the continence service and the development of skills in continence within the wider CFHS organisation and allied organisations, to improve the knowledge and skills of all those involve with CYP in relation to continence.

    5. Be available to provide support, teaching and advice to other members of CFHS, in particular to 0-19 teams and Special School Nurses.

    6. Provide an annual update of training in daytime enuresis assessment and management.

    7. Act as a mentor for student nurse learners (HE) and other college (FE) students who are placed in the team as part of their education.

    8. Maintain own knowledge and skills through continuing professional development and share knowledge, expertise and act as a resource to colleagues and other health and social care workers.

    9. Be responsible for ensuring that Assistant Practitioners within the team access Clinical Supervision.

    10. Facilitate team cohesion through being a responsive, pro-active and flexible practitioner.

    12. Participate in the organisations Performance Appraisal Scheme and develop close working relationship with Practice Development Facilitator, 0-19 practice teachers and Special School Nurses across Surrey.

    13. Support learning by recognition of own and others learning needs, interests and styles.

    14. Gain feedback from learners and relevant others on the effectiveness of learning and development and ideas for how it might be improved. Reflect on and evaluate the effectiveness of learning.

    15. Discuss own evaluation with the team and agree how learning and development might be improved in the future.

    1. Provide accurate data about activity and caseload analysis as required.

    2. Ensure personal documentation, client and staff records are maintained effectively whilst being aware of the legislation around client confidentiality and following organisational policies and procedures.
