    TrinityHealth PACE at Saint Francis LIFE has an excellent fulltimeemployment opportunity for a Board Certified Family Medicine orInternal Medicine physician with fellowship training or Certificateof Added Qualifications in Geriatric Medicine to become MedicalDirector of a Program of AllInclusive Care for the Elderly (PACE)located in WilmingtonDE.

    JobDescription:

  • Underthe supervision of the ExecutiveDirector.
  • Provides general medicaldirection and supervision of the medical aspects of participantcare
  • Responsible for the delivery ofparticipant care clinical outcomes and oversight of the QualityAssurance Performance Improvement (QAPI) program.
  • Responsible for the development andmaintenance of medical policies and procedures/standards andprotocols
  • Provides direction and supervision torelated Primary Care Providers (PCPs) andAPPs.
  • Provides leadership and guidance to theprogram colleagues and providers in the localcommunity
  • Need to have a driverslicense proof of insurance and means oftransportation
  • Proficient inMicrosoft Suite (Word Excel PowerPoint etc.) in addition to basicknowledge of email calendaring systems and other software asrequired to perform the essential functions of thejob
  • Excellent organizational customer serviceflexibility and problemsolvingskills
  • Willingness to travel within the regionperiodically as needed
  • Conduct history andphysical on new participants and eventbased and periodicreassessments. Develop medical treatment plan and provide medicallynecessary services in the range of locations in which participantsmay reside.
  • Interview and evaluate applicantsto PACE Organization and their caregivers and participate in theIntake and Assessment decision.
  • Discuss eachparticipants medical conditions and prognosis with him/her andtheir caregiver to assist them in expressing their wishes andplanning their future care.
  • Execute assignmentsin a culture that is shared and collaborative across all divisionswithin TH PACE.
  • Collaborate in the nursingtreatment program of participants.
  • Establishand/or maintain as a member of the leadership team a plan toachieve operating goals.
  • Participate as anactive member of PACE Organization interdisciplinary team. Educatesand advises the team regarding the implications of diagnosescognitive and physical function and treatmentoptions.
  • Participate in team and familyconferences and decisionmakingprocesses.
  • Educate participants and familiesregarding health maintenance and their chronic medical conditionsin cooperation with nursing staff.
  • Supervisethe health care and treatment program as developed by theinterdisciplinary team.
  • Perform physicalexaminations order and evaluate appropriate laboratory anddiagnostic tests and record results in a systematicmanner.
  • Effective skills in physical assessmentand chronic disease management for frail olderadults.
  • Desire to work in a creative alternatehealth care setting.
  • Able to manage changingpriorities according to participantneeds.
  • Participates in provider oncallrotation asrequired.
  • Minimumof 1 year experience working with the frail or elderly populationisrequired.
