    Job summary

    An excellent oppertunity has arisen for an ANP to join our friendly practice to help us deliver high quality care.

    Main duties of the job

    We are looking for a part time ANP for 18-27 hours per week. The post holder will need to triage the needs of the presenting patient from taking medical history, assessment, diagnosis, and treatment ensuring any necessary referrals are made whilst acting within their professional boundaries.

    The post holder must be a registered general nurse (currently registered with the Nursing and Midwifery Council), have a recognised NP qialification (minium of degree level or equivalent) and also be an independant Nurse Prescriber.

    Our Practice consists of 2 GP Partners, 1 Practice Manager, 1 ANP, 2 Practice Nurses, HCA and a competent administration team.

    Job description

    Job responsibilities

    As an ANP youwill be an experienced clinician will provide acute same day care to patientsregistered at the surgery. This includes telephone triage and F2F assessments and also home visits.

    Key Responsibilities

    The Advanced Nurse Practitioner works autonomously and isaccountable for own professional actions.

    Assess, diagnose, plan & implement treatment/interventionsand care for patients.

    Refers patients to other medical specialities for assessment ifdeemed appropriate.

    Diagnose and manage both acute and chronic conditions

    Prescribe and review medication for therapeutic effectiveness inaccordance with evidence-based practice and national and practice protocols,and within scope of practice.

    Work with patients in order to support compliance withprescribed treatments.

    Communicates highly sensitive condition related information topatients, relatives and carers. Initiates the process of diagnosis withpatients suspected to have a chronic diseases eg. chronic diseases, within thedisease management agenda eg QOF, IIF.

    Undertakes annual review of patients understanding and abilityto self-manage and medication reviews within scope of competency.

    To actively implement safeguarding protocols for children andvulnerable adults and to know who to refer to and how to escalate concerns
