    An exciting opportunity has arisen for the position of Integrated Community Services Manager.

    The successful applicant will be responsible for the development of integrated community services to meet the health and social care needs of the population it serves.

    The post holder will be required to manage our integrated Cluster Network Teams and Long-Term services.

    This is a unique and exciting opportunity for a registered Social Care and/or Health professional to join the senior management team for the Integrated Community Health and Social Care service in Bridgend.

    As an inspirational senior leader, you will be required to direct, lead, and motivate the Integrated Cluster Network Services to ensure a high standard of professionalism, efficiency, effectiveness, and innovation in the delivery of strength based person-centred care and support, working collaboratively with primary care and mental health services.

    You will work with the other Integrated service teams, and professionals across the region to ensure consistency of approach and roll out of best practice and deliver seamless pathways of care
    The postholder will ensure that there is a robust assessment and decision-making processes in place, as well as high quality service delivery within the Primary Community & Mental Health Care Group (PC&MH) /County Borough Council, in line with University Health Board and Local Authority policies and frameworks, supported by robust information and analysis for planning purposes

    You will be expected to work independently and be guided by national and local health and social care policies, guidance, and strategies, advising the Local Authority and the UHB PC&MH Care Group on how these should be interpreted and implemented locally.

    Intrinsic to this role is working with the Heads of different professions that support the Integrated Cluster Network Services, to ensure high professional standards of service delivery, robust relationships to the professional departments, that will then support consistent professional leadership, practice and guidance
    In addition, you will promote awareness of, and the benefits of Integrated Community, Frailty, and complex care services, to relevant stakeholders interested groups partners and other agencies

    Experience at a senior level within either local authority, the NHS, or in the voluntary or private sector is required.
