- Conduct routine clinics for immunisations, cervical screening, wound management, and chronic disease management such as asthma, diabetes, and hypertension.
- Perform initial patient assessments, including taking medical histories and vital signs.
- Implement and evaluate individual care plans for patients.
- Provide health education, advice, and support to patients and their families.
- Assist in the management and administration of medications as per protocol.
- Collaborate with other members of the healthcare team to facilitate comprehensive care delivery.
- Maintain accurate and detailed clinical records using electronic health record systems.
- Participate in quality improvement initiatives within the practice.
- Registered Nurse (RN) with current NMC registration.
- A minimum of 2 years of nursing experience in a GP surgery or similar setting within the UK.
- Proficient in using clinical software systems.
- Excellent communication and interpersonal skills.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Experience in chronic disease management and routine clinical procedures.
