    £30 to £35 per hour

    About the Role:

    We are seeking a dedicated and experienced Practice Nurse to join our dynamic team at a busy GP Surgery located in Reading. This part-time position is ideal for someone who is passionate about providing exceptional patient care within a community-focused environment. The successful candidate will play a crucial role in the delivery of primary care services and will work collaboratively with GPs and other healthcare professionals to ensure the highest standards of care are maintained.

    Key Responsibilities:

    • Conduct routine clinics for immunisations, cervical screening, wound management, and chronic disease management such as asthma, diabetes, and hypertension.
    • Perform initial patient assessments, including taking medical histories and vital signs.
    • Implement and evaluate individual care plans for patients.
    • Provide health education, advice, and support to patients and their families.
    • Assist in the management and administration of medications as per protocol.
    • Collaborate with other members of the healthcare team to facilitate comprehensive care delivery.
    • Maintain accurate and detailed clinical records using electronic health record systems.
    • Participate in quality improvement initiatives within the practice.

    Qualifications & Experience:

    • Registered Nurse (RN) with current NMC registration.
    • A minimum of 2 years of nursing experience in a GP surgery or similar setting within the UK.
    • Proficient in using clinical software systems.
    • Excellent communication and interpersonal skills.
    • Ability to work effectively both independently and as part of a multidisciplinary team.
    • Experience in chronic disease management and routine clinical procedures.
    • Commitment to ongoing professional development and training.
