    An exciting opportunity has arisen within our Out of Hospital Division for a General Manager for South Warwickshire.     Reporting to the Associate Chief Operating Officer for Out of Hospital, this full time, permanent post is available on our Senior Manager Trust Terms & Conditions. You will be educated to master's degree level or equivalent with significant experience working in a senior management position in health care and / or social care.

    Although this role is not a formal joint appointment between health and social care, this role will work across both to shape the planning and delivery of health and care services across the county, recognising the differences between the three Warwickshire places and their population needs.

    There is a strong emphasis on our transformation programme, focused on getting the right care to patients, at the right time, in the right place, with a mandate to only provide care in the acute setting that cannot be delivered elsewhere. Local partnership arrangements are strong, aiming to align acute, community, primary and social care services.

    There is a requirement to take part in the manager on call rota.

    Main duties of the job

    The post holder will be a Senior Manager with responsibility for the operational management and delivery of Adult Community Services at South Place. Working closely with General Managers in Rugby and North Warwickshire to deliver ambitions for Warwickshire residents specifically the configuration of Community Services to keep people healthy and well at home for as long as possible.

    The role is responsible for delivery of Out of Hospital contractual expectations at Place and agreed Outcomes.

    The General Manager (GM) will balance a number of factors in order to provide a successful service:

    Outcomes and quality of service issues

    Ensure that Out of Hospital contractual outcomes are achieved and sustained.

    Ensure the optimum management of Community Services resources in North Warwickshire Place so that clinical staff can carry out their functions effectively.

    Ensure that agreed Outcomes are achieved, ensuring that access and response targets are met. Identify potential problems and variances against agreed targets at the earliest opportunity and be instrumental in corrective action.

    Lead the development of strategic and business planning objectives for Warwickshire North Place ensuring the involvement and commitment of key staff members to the process, promoting a culture in which staff are engaged and empowered to design services to improve outcomes, responsiveness, and efficiency.

    Manage the performance of the Warwickshire North Place against agreed national/regional quality standards adopted by SWFT, GEH and partners.

    Manage the service development plans, including providing leadership for the development of business cases, service reviews and the delivery of service changes.

    Ensure that patient-focused clinical services are developed in line with Trust Strategy, NHS Plan National and other national initiatives.

    Activity and quality of service issues

    To ensure that performance information is maintained, monitored and evaluated to ensure sufficient capacity to support delivery of robust and sustainable Adult Community Services.

    To ensure that monitoring and action plans are in place to maintain and update required registrations under the Care Quality Commission (CQC) frameworks in delivery of Outstanding services.

    To embed prevention, reablement and early support ethos and principles of Community resilience across Out of Hospital pathways.

    To develop business cases to deliver sustainable Adult Out of Hospital services to include citizens with on-going issues such as mental health care needs, dementia, stroke, learning disability etc.

    To work with local communities and the voluntary sector to create appropriate community support schemes.

    To work in collaboration with internal and external partners to ensure integrated services are developed and successfully delivered.

    Finance

    To have overall responsibility for all budgets associated with the services, ensuring that resources are used effectively, with no duplication, no unnecessary handovers between service pathways and that balanced budgets are delivered at year end.

    Manage the financial performance at Warwickshire North Place highlighting potential problems at the earliest opportunity and be instrumental in implementing corrective action.

    Ensure that all resource implications and changes in services within Warwickshire North Place are clearly communicated as part of business and financial planning cycle.

    Support the Operational Management Team and Divisional Finance Manager in the negotiation and agreement of Place budgets, agreeing with the Associate Chief Operating Officer of Out of Hospital Care Collaborative appropriate delegation of functional budget to self- managed teams.

    Manage the performance of the Warwickshire North Place to achieve effectiveness and efficiency targets including Cost Improvement Programmes.

    Be responsible for the budgetary management of Capital Projects as directed by Associate Director of Operations, Out of Hospital Care Collaborative.

    Information and Communication

    Take a lead in the implementation of Adult Out of Hospital Care Collaborative communication strategies in relation to Warwickshire North Place.

    Ensure that methods of organisation and sources of information are patient centred.

    Communicate with other appropriate professional bodies, locally and nationally, to promote and improve the services in line with the Trust's objectives and nationally agreed initiatives.

    Ensure that communication systems support the effective management of service delivery and ensure that staff and stakeholder views are actively sought.

    Ensure flexible and imaginative feedback systems exist to receive user and carer views on the running of current services, as well as planning those for the future.

    General Management

    To participate in the on-call rota for the management of SWFT.

    Ensure working hours of staff under your responsibility are managed in order to maintain a healthy work/life balance and comply with the EWTD.

    Ensure that the Trust's Complaints Procedure is adhered to, responding to complaints/criticisms as required and ensuring that supporting documentation is made available as appropriate, particularly in cases of litigation.

    Represent the Out of Hospital Care Collaborative and be actively involved in SWFT Trustwide teams/committees as appropriate.

    Represent the Out of Hospital Care Collaborative and be actively involved in GEH Trustwide teams/committees as appropriate.

    Ensure delivery of the SWFT and GEH Trust governance agenda, including Risk Management, Health and Safety and quality standards, through the development, implementation and review of governance systems to ensure a safe clinical and working environment.

    Ensure that all Trust standards are maintained and monitored to improve the quality of care to all who come into contact with services provided by SWFT and GEH.

    Working Relationships

    The General Manager will work in close partnership with SWFT and GEH leadershipteams to support the development of integrated service delivery plans for North Warwickshire Place in line with the Integrated Care System strategy and priorities.

    The General Manager will have close working relationships with:

    Associate Medical Directors, Public Health Consultants, other medical staff, senior nurses, Allied Health Professionals and Managers across both GEH and SWFT.

    Local NHS Trusts, including the ICB, Hospital and other healthcare providers

    Primary Care and GP Federations

    The post holder to engage with clinical networks and patient organisations to take forward Out of Hospital Programme.
