Description
Leadership To participate in clinical supervision. To participate with the manager in personal appraisal to identify professional and personal development. To participate in relevant training and take personal responsibility to keep professionally up to date with practice. To appropriately delegate care to members of the multi skilled team ensuring oversight of care delivered and supervision.


To take on the role of clinical supervisor, coach and mentor for peers, colleagues, students and other staff as appropriate.

Clinical To encourage appropriate access to Primary Care Services for homeless, traveller and asylum households To maintain clinical competency and a portfolio of evidence of continuing professional development.

To comply with NMC guidelines and relevant legislation to ensure that clinical practice is of the highest standard. To prescribe medicines as appropriate in accordance with the NMC prescribing standards within the Nurse Prescribing Formulary.


To manage time effectively, prioritise own workload and supervise the practice of other team members (including bank and agency staff), both directly and indirectly as appropriate.

Communications To maintain accurate, legible and contemporaneous electronic records in accordance with Trust and NMC guidelines and policies. To liaise effectively with other members of the team and other relevant agencies. To make ongoing referrals for support as necessary working with other care providers.

To maintain effective working relationships with social care, safeguarding services and childrens centers.

To maintain effective communications systems with Health Visiting and School Nursing teams and Primary and Secondary Care teams in order to facilitate continuity of care for individual patients.

To participate in team meetings and performance reviews in order to contribute to the teams objectives for service, staff and personal development.

To feedback relevant issues regarding resourcing, workload and training to team meetings.

Safeguarding To maintain and update knowledge around safeguarding children and young people. To ensure that practice complies with local and national policies and legislation. To act as the Lead Professional for vulnerable children when necessary. To attend and to be an active participant in Child in Need meetings, Case Conferences and Core Groups.

To produce reports and convene Core Groups and Child in Need meetings as appropriate. To attend regular safeguarding supervision and to maintain close working relationships with the Safeguarding Advisory Service.

Education and Training To assist in the preparation and delivery of teaching programmes and presentations for students, staff and others as the need arises.

Clinical Governance and Development To carry out risk assessments, ensure a safe working environment and provide team members and partnership organisations with information relevant to their safety.


To report adverse incidents within the service and support staff in completing incident reports and developing action plans as appropriate.

To contribute to the development of the Clinical Governance plan for the service and translate actions into practice ensuring safe practice To participate in clinical audit, screening programmes and research as necessary.

To draw on evidence based practice as a source for giving and developing care and extending the homeless/traveller/asylum care specialist knowledge base.

To monitor and evaluate the performance and the standard of service delivery.


Service development You will contribute to the development of the service for Homeless people, Gypsies and Travellers and Asylum Seekers and Refugees.

To contribute to the development of policy taking a lead in agreed specific areas on behalf of colleagues and the organisation as appropriate To encourage research and evidence based practice within the team.

To actively participate in and facilitate health promotion programmes. To participate in clinical audit, screening programmes and research as necessary.

To draw on evidence based practice as a source for giving and developing care and extending the homeless/traveller/asylum care specialist knowledge base.
