    Job summary

    Do you have the drive and ambition to move patient pathways forward? In your role as a Patient Pathway Coordinator, you will be a key contributor to patients receiving care as quickly as possible. This is a rewarding role where you will help enable joined up care for our patients from referral to treatment and/or discharge and support the achievement of RTT and cancer targets. You will be joining a friendly team that work well together in our vision to always provide excellent care with compassion to our patients.

    We are looking for a proactive and motivated individual who enjoys working in a challenging and busy environment to support the Upper GI and Colorectal SBU. You will be part of a team helping to improve our patients' experiences and reduce our waiting lists for treatment.

    It is important that we get the right person who can provide effective cover across the team, so we are looking for someone with the right behaviours, skills and willingness to learn.

    Main duties of the job

    You'll be at the forefront of supporting the achievement of our referral to treatment targets, including crucial national cancer goals. From scheduling appointments to coordinating treatments, your efforts will directly impact patients' journeys from referral to recovery.

    As a key player in our Colorectal and Upper GI departments, you'll be instrumental in managing patient pathways effectively. From administrative tasks to tracking progress, you'll collaborate closely with various team members to ensure seamless care within target timeframes. Your role will involve generating insightful weekly reports to keep everyone informed and actively monitoring potential risks, ensuring proactive steps are taken to address them.

    Qualifications & Education

    Essential

  • English and Maths GCSE (Grade C or above) or equivalent
  • Knowledge & Experience

    Essential

  • Experience of working in a busy NHS environment
  • A good understanding of clinical services within a hospital setting.
  • Awareness/understanding of 18 week RTT Targets
  • Knowledge of patient pathways from referral to discharge
  • Desirable

  • Evidence of improving patient pathways/ reducing waiting times for patients
  • Proven experience in successfully managing both National and Local KPI's
  • Proven ability in clinic management in order to ensure capacity meets demand together with the ability to think creatively in ways of delivering the required capacity
  • Skills and Abilities

    Essential

  • Sound understanding and operational experience of Microsoft Windows, Microsoft Office 365, Excel and Databases.
  • Excellent attention to detail and accuracy in the production of work.
  • Experience of monitoring and auditing work processes.
  • Able to grasp new concepts quickly and produce timely written reports at short notice
  • Able to establish rapport and good working relationships with a wide range of multi-disciplinary staff groups.
  • Proven verbal and written communication skills.
  • Able to work independently or as part of a team.
  • Investigative and analytical approach.
  • Desirable

  • A sound understanding and operational experience of specialist clinical systems used in health care.
  • Proven ability to speak confidently, empathetically and professionally to patients via telephone
