To work clinically as a urology nurse specialistproviding a range of nurse led clinics, support the service and support thedepartment to develop your knowledge and skills. Identifying and directingpatient referrals from the GP and other sources to the most appropriate clinic.To provide a high standard of evidenced based care for patients with urologicalconditions. To provide teaching and develop clinical expertise across thehospital sites and community teams with a passion for improving quality. Wecurrently are able to offer a 30 hour post.

  • To liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries. For both benign and cancer urological conditions.
  • To undertake nurse led clinics and to be responsible for excellent standards of nursing care to patients through the planning, co-ordinating, delivering and evaluating nurse-led clinics.
  • To work autonomously to deliver specialist clinical care appropriate to the needs of the patient group and based on principles that are research based and which enhance the quality of patient care.
  • To contribute to the facilitation of the planning, delivery and evaluation of care pathways for a designated group of patients.
  • To contribute to training and education within specialist area.
  • To promote an innovative and progressive attitude to the continual improvement of patient care through reviewing the evidence base and embedding this in practice through engagement in research activities.

    Job responsibilities

    The CNSs will workalongside Consultant Urologists in supporting patients referred to the Trustwith both benign and suspected cancers on different pathways. Through theprinciples of the Best Practice Prostate Pathway, it is anticipated thaturology patients on these pathways will have an improved waiting time todiagnosis with a positive patient experience. The CNS will also work along siteConsultants and support the Urology cancer and benign services.

    The post holderswill work closely with the Consultant Urologists in delivering pathways forbenign service and suspected urological cancers. This job description outlinesthe current main responsibilities of the post. However, the duties of the postmay change and develop over time and this job description may, therefore beamended in consultation with the post holder.

    The CNS willdevelop to work effectively as an autonomous practitioner and as a core memberof the multi-disciplinary team within Urology to provide the highest possiblestandards of care for patients with urological disease.

    The CNS will workwith the support and direction of the head of nursing for surgical specialties,the matron, the Trust lead cancer nurse and the Consultant urologists toimprove the care of patients, provide specialist help advice and support topatients and their families. You will act as an advocate by providinginformation which leads to informing patient choice. You will use the views ofpatients and carers to inform service improvement.

    The post holderwill develop and work as an integral member of the Multidisciplinary Teamacross the Worcestershire Acute Hospitals, collaborating with a range of healthcare providers. The post holder will provide highly developed specialistnursing, Education, Research and Audit, Leadership and Consultancy and aProfessional Role model.

    EXPERT CLINICALPRACTICE:

    1. Will develop toprovide specialist nursing practice intervention so that the specific needs ofindividual patients / clients are addressed. Provide direct care as per job plan,working alongside the nursing staff in the clinical area- providing a visibleand strong role model to ensure best practice

    2. Will develop toconduct independent nurse led clinics for a variety of urological conditionsthat need monitoring/treatment or review post-surgery. Work autonomously withinthe parameters of their competencies to make decisions and adjustments topatients treatments in line with their needs. Will develop skills that mayinclude, altering medication, requesting imaging and appropriate blood tests aswell as referring back to the consultant or other multi-disciplinary members iffurther advice is needed.

    3. To provideadvice and support for all members of the multi- professional team in the careof patients undergoing diagnostic pathway with suspected urological cancer. Toliaise closely with members of the multi professional team so that patientswith confirmed urology cancer experience a seamless, high quality service.

    4. To providecontinence advice.

    5. To provideguidance, support, advice and counselling to patients and their family/ carersalong the patients pathway. To know when to refer on to other professionals formore specialist guidance, support, advice and counselling.

    6. To promotepatients autonomy through education and support so that the individual is ableto make informed choices about their own care.

    7. To act as thepatients advocate when required and liaise with the multidisciplinary team asnecessary.

    8. To act as a rolemodel of expert clinical practice, empowering others through effective rolemodelling, teaching, and team working.

    9. To developguidelines and protocols to deliver comprehensive evidence-based service forpatients.

    10. To recogniseopportunities within the urology service for further development and expansion,to facilitate and manage change within defined clinical and organisationalboundaries.

    11. Working as anintegral part of the multidisciplinary team to develop the service andappropriate referrals. To develop effective working relationships with allother departments resulting in effective coordinated care for clients, with allother local, regional and national organisations

    12. To be able towork both individually and as part of the multidisciplinary team.

    13. To case managea team of patients in partnership with the multi-professional team.

    14. To set, monitorand review standards on a regular basis in accordance with clinical governanceessence of care benchmarks and the NHS plan. To liaise with MDT and Matron re:leading on patients satisfaction surveys and present findings at ClinicalGovernance and other appropriate meetings.

    15. Develop skillsthat enable the CNS to lead on developing integrated care pathways and nursingcare pathways in liaison with members of the multidisciplinary team.

    16. To assist inthe development own nurse led clinics in conjunction with the Consultants andthe outpatients department, after consultation with the Matron and keystakeholders.

    17. To maintainnurse led and telephone clinics whereby patients are provided with informationand support and the problems and needs of patients and their careers areassessed and appropriately channelled and referred to other services asrequired.

    18. Incollaboration with patient information services write new patient informationand update existing patient information.

    EDUCATOR

    1. To identifyassess and meet the educational needs of the patients and their family

    2. To identify theeducational needs of the multi-professional team and to facilitate andparticipate in education provision to meet those needs. To actively teach thenursing staff in the clinical area and evaluate their learning in conjunctionwith the senior nursing staff

    3. To workcollaboratively on educational strategies which support urology cancer servicesand service development

    4. To develop anddeliver education programmes on the ward in liaison with the multidisciplinaryteam/nursing staff.
