An exciting opportunity has arisen for two Speech and Language Therapists to work for Norfolk Community Health and Care, providing Speech and Language Therapy in the Early Supported Discharge Stroke Team and Beech inpatient stroke rehabilitation unit, which is part of the Stroke Service.

The post holders will work with adults with acquired speech and language difficulties and dysphagia related to stroke. The roles will be based at Norwich Community Hospital and in patient's homes.

The posts will suit someone who is passionate about stroke rehabilitation, setting and working on goals to enhance the life of stroke patients and working closely in a multi-disciplinary team.

Regular team meetings, support from Specialist Therapists and Nurses and supervision will ensure the post holders development and provision of a high quality of service.


  • To provide high quality Stroke Specialist Speech and Language Therapy assessment, differential diagnosis and treatment for adults with communication and swallowing disorders following a stroke.
  • To work collaboratively across the Central Norfolk Integrated Stroke Service to ensure a seamless provision of stroke services across the acute hospital, inpatient rehabilitation and early supported discharge teams.
  • To work on a rotational basis across Beech Ward and Early Supported Discharge, typically on a six month rotation but depending on needs of services.
  • To develop targeted therapy programmes to be carried out by Speech and Language Therapists and Rehabilitation Assistants / Assistant Practitioners.
  • To work as part of an interdisciplinary team to ensure interprofessional patient care for patients with communication and swallowing disorders.
  • To contribute to service development and data collection within Central Norfolk Integrated Stroke Services.

  • To carry out the assessment, implementation and evaluation of evidence based specialist treatment programs for patients following a stroke, who may have a range of impairments and complex health needs.
  • To be professionally and legally accountable for all clinical decisions and professional actions and to ensure that work is within professional standards, clinical guidelines and in line with evidence based practice.
  • To provide assessment and clearly planned SLT intervention for patients with a communication and swallowing disorders following a stroke.
  • To devise, monitor, supervise and evaluate the implementation of communication and swallowing intervention programmes carried out by others.
  • To sensitively negotiate patientled goals with patients, families, carers and the interdisciplinary team, in order to enable patients to engage in treatment, and to ensure that patients are involved in the planning and prioritisation of their care plans where possible.
