Main Duties and Responsibilities 1.

The Unit Lead is integral to the delivery of safe, effective, consistent and high-quality admissions, transfer and discharge services for patients and their care givers in a reablement setting.

To provide senior leadership and lead on the development of a therapy led, reablement culture within the new unit and to ensure that the ethos becomes part of the culture within the teams.


  • To lead a multidisciplinary team within a quality framework to ensure that the highest standards of care are met. 4. To lead and support the Clinical and Therapy Leads to lead and support the Clinical and Therapy staff, Discharge Coordinators, Activities Co-ordinators, Therapy Assistants and administrator(s).
  • To create an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients and their cares and families.6. Work collaboratively with Health and Social Care professionals to maximise admission avoidance opportunities and support a HomeFirst ethos to discharge patients. Lead on capacity or patient flow issues and the effective implementation of escalation procedures when the whole system or one constituent part of the system is unable to manage the presented demand being placed upon it.
