    We are looking to recruit a Band 5 community staff nurse into the Romsey Integrated Care Team.

    You can make that difference and contribute to the provision of high-quality care, within a variety of community settings which include the patient's home, IV therapy and catheter clinics.

    Learn new skills including end of life care, wound care and holistic care planning.

    The multi-disciplinary Integrated Community Teams work alongside primary care and a variety of community services to support and manage the care of housebound and high-risk patients.

    The scope of your role will focus on supporting admission avoidance, early supported discharge, promotion of independence and assisting in the effective management of patients with long term conditions.

    The service runs from 8am-8pm, 7 days a week, , and therefore this position requires flexible working, but come along and ask us - what hours would suit you?
    All applicants require a driving license, professional registration and full access to a vehicle for travelling (mileage allowance payable)
    Jackie Arnold Integrated Community Lead

    Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, mental health, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.

    The post holder will have responsibility for managing, co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.

    A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community.

    The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.

    Registered Qualification (RGN/RMN/PT/OTA)
    Current (UK/EU) Driving Licence

    Experience of planning person centred needsExperience of organising resources relating to patient needs (e.g. equipment, dressings, etc.)Participate in the holistic assessment, implementation and evaluation of programmes of care for patients in the home, without direct supervision.

    This includes those with chronic disease, terminal illness who wish to die at home and complex care needs for example the physically disabled.

    Other settings include residential homes, nursing homes, clinics, community centres and community hospitals.

    For those staff that have a dual role within practice in the ICT that extends to general practice working.

    Maintain upheld and follow the standards within the NMC code of conduct at all times ensure safe and effective delivery of patient care.

    To remain up to date and within guidelines for revalidation within NMC code of conduct at all times.

    Review effectiveness of nursing care provided ensuring that patient safety is maintained and care delivery meets the standards required by the Trust and Care Quality Commission.

    Advise on the promotion of health and prevention of illness and provide information to individuals and groups to prevent disease, where possible.

    Recognise situations that may be detrimental to health for example housing, social and economic factors and refer to an appropriate agency and liaise with members of the Community Care Team.

    Maintain clear and accurate records of care in line with standards required by the Trust and NMC.
