Core Purpose:To work as part of a team to provide specialist support for young carers and young adult carers.


Key Responsibilities

  • Manage a caseload of service users as allocated by the Lead Practitioner or Team Manager and complete high quality and comprehensive assessments and risk assessments, including liaising with family members, significant others and professionals / agencies as appropriate.
  • Develop comprehensive support plans to identify how individual service user needs will be met in order to help reduce the impact of caring responsibilities for each service user and achieve sustained outcomes.
  • Review individual support plans within appropriate timescales and no later than once every three months.
  • Deliver support sessions to small groups of service users and, where appropriate, their families, to meet their specific needs.
  • Support the development, coordination and delivery of activities for young carers and young adult carers and, where appropriate, their families.
  • Deliver sessions to professionals to raise awareness of young carers and young adult carers
issues and the service, to encourage referrals and enable the young carers and young adult

carers to be supported in their caring role.

  • Record case notes, input information onto the database promptly and maintain other relevant
  • Management Information Systems as required in order to ensure effective case management and assist monitoring and evaluation of service delivery.
  • Transport service users on an adhoc basis to enable them to access appointments, groups and activities.
  • Work as part of a team providing cross provision/ service support where reasonable and attending regular team meetings.
  • Undertake continuing professional development including participating in supervision, performance development reviews and attending training as/when required.
  • Work in accordance with all relevant legislation, policies & procedures and guidelines both
internal and external.

  • Maintain accurate and up to date records and reports and provide written and verbal reports as required.
  • Keep abreast of policy and professional development within your area of professional expertise.
  • Work flexibly across operational sites and within an agreed number of hours to maintain
appropriate service provision. This will include evening and weekend working.

Essential Criteria:


  • At least 2 years experience in teaching ESOL
  • Appropriate teaching qualification e.g. PTLLS, CTLLS, DTLLS, Cert Ed, PGCE
  • Knowledge and experience of adult education
  • Knowledge and experience of Safeguarding and Prevent
  • Experience of effective personalised teaching and learning for learners experiencing diverse barriers
  • Experience of successful teaching of accredited qualifications
  • Experience of developing and creating learning materials and resources
  • Flexibility across geographical locations
  • Clean driving license and fulltime access to car

Salary:
Up to £25,000.00 per year


Schedule:

  • 8 hour shift
  • Monday to Friday
