This is an exciting opportunity for an experienced individual to undertake the role of Independence Advisor in the new Preparing for Adulthood service.

The service is located within Adult Social Care but will work across both Children's and Adults' services, and aims to improve the timeliness and quality of support offered to young people on their journey into adulthood.


You will work alongside young people, parent carers and a range of professionals across both Children's and Adults' Service to ensure there is a coordinated and timely approach to assessing the needs of young people prior to their 18th birthday, and co-producing post-18 care and support plans so that young people are prepared and supported on their journey.


You will have experience of working within Adult Social Care where you will have undertaken Care Act assessments, developed person-centred and strengths-based care and support plans, as well as having had some experience of supporting young people in their transition to adulthood.


You will have a good working knowledge and understanding of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, Children and Families Act 2014 and the Preparing for Adulthood outcomes framework, as well as the principles underpinning a person-centred and strengths-based approach to assessment and planning activities.
