- We are looking for an enthusiastic administrator who has excellent organisational, multidisciplinary and communication skills with experience working in the general practice setting.
- Provide coordination and navigation to patients across the organisation requiring medication support through processing medication requests, shared care monitoring and carrying out Medicines Management projects.

Main duties of the job
Aims:
Determine accuracy of both hospital and GP prescriptions with regard to medication
Provide independent information to patients with regard to medicines management and through assessment initiate further support/input from other healthcare professionals where appropriate
Implement procedural and service changes relating to prescribing to GP practice staff to ensure best practice is adopted in accordance with local guidelines and legislation
To link into local clinical governance structures to report on risk management issues relating to the prescribing process
To support the surgery in the planning and delivery of medicines management services
Daily Nursing home Prescriptions monitoring prescription requests
Appliance/wound care contractor request monitoring
Prescription queries from healthcare professionals / More complex queries from healthcare professionals
Daily queries/information requests from clinicians
Daily queries from all outside agencies including patients
Discharge summaries - change of medication, adding new medication before checked by the clinicians and inform necessary people eg. Patients, Nursing/care homes, Pharmacies and where needed blister pack patients
Action any new guidelines from LMMG/CCG

Service Delivery
Staff at The Riverside MedicalCentre must adhere to the information contained with the policies and regionaldirectives, ensuring protocols are adhered to at all times. Staff will be givendetailed information during the induction process regarding policy andprocedure.
Security
The security of the site of workis the responsibility of all personnel. Staff must ensure they remain vigilantat all times and report any suspicious activity immediately to their linemanager. Under no circumstances are staffs to share the codes for the doorlocks to anyone and are to ensure that restricted areas remain effectivelysecured.
Professional Conduct
At The Riverside Medical Centre,staffs are required to dress appropriately for their role.
Leave
All personnel are entitled totake leave. Line managers are to ensure all of their staff are afforded theopportunity to take a minimum of four weeks plus bank holiday days leave eachyear(pro rota for part time), and should be encouraged to take all of theirleave entitlement.
