It is an extremely exciting time here at Cephas Care with the recent implementation of a new senior management team devoted to providing service excellence and supporting our staff.

It is a great opportunity to join our team, the Cephas Care Group are looking for highly motivated, enthusiastic, and experienced Team Leaders to work in our projects across Suffolk.


Hourly Rate:
£12.00 per hour
.

Hours:40 hours per week, this will be inclusive of weekend work.


Objective of Role:


You will predominantly be working in the field within our learning disabilities projects based in Suffolk, you will need to plan, implement, and assist the Registered Manager in the full operational function of each learning disability service.

You will provide inspirational leadership, managing and motivating a team to provide a quality, person-centered specialist service, delivering outstanding levels of care and support to our service users.


Main Duties:


  • Be responsible for the daytoday operational case management, assessment, and delivery of specialist social care interventions for adults with learning disabilities operating within a culture of continuous service improvement and safety to ensure professional standards and best practice are embedded throughout the team.
  • Provide excellent leadership, direction, and effective management of resources, ensuring all team members understand, embrace and deliver to the strategic aims of the service.
  • Be responsible for all aspects of management, development, and leadership within the service area. This includes responsibility for effective communication and information sharing as well as dealing with all personnel and performance issues
  • Actively manage, supervise, and participate in the delivery of care to the service users promoting independence, inclusion, dignity, choice, and respect.
  • Promote the image, reputation, and best interests of Cephas Care.
  • Ensure service users choice and independence is respected through risk management involving all parties and ensuring accurate up to date record keeping and production of risk assessments.
  • Respond to emergencies and provide appropriate guidance and support to all staff in the event of the same.
  • Be responsible for reviews and updating all service user care plans, whilst being responsible for continual monitoring of each service user's health needs liaising with all relevant persons/agencies.
  • Be familiar with the administrative records of the service ensuring all records are kept up to date.
  • Welcome service users to be involved in the service and ensure the staff team encourage their participation in activities and foster emotional and intellectual stimulation. Manage as required social functions taking place at the service.
  • Support and work closely with families and friends.
  • Complete regular medication audits, staff medication competencies and ensure the accuracy of MAR chart production, use and completion.
  • To complete regular, effective supervision and appraisal with staff and support with identified training needs to engage and motivate the staff team so as to ensure positive outcomes for all service users.
  • Effectively lead, motivate and support staff, assisting in appropriate induction and support of new staff to the service.
  • Complete staff rostering and to ensure that the services have consistent staffing to ensure that the needs of all service users are met.
  • Improve choice and quality of life for through the use of timely interventions and/or review and reassessment that maximise longterm independence and minimise ongoing support.
  • Hold quarterly tenant and relative meetings, minuting the meetings, developing actions, and producing outcomes.

Person Specification:


  • Hold a relevant professional qualification for social care.
  • Experience leading a health or social care team.
  • Management of budgets.
  • Effective communication including written, verbal and presentation skills.
  • Clear and thorough report writing.
  • Contribution to service planning.
  • The ability to maintain appropriate records in accordance with the legislation requirements relating to access to files.
  • Record of high performance in successfully delivering a customer focused service to demanding targets and objectives.
  • Taking a lead with adults at risk where there are safeguarding concerns.
  • Working in partnership with internal and external providers
  • Sound, practical understanding of relevant legislation (such as the Mental Capacity Act and the Community Care Act), policies and processes necessary to deliver complex services to vulnerable adults.
