    Job summary

    The post holder will assess patient's fitness for surgery prior to elective ophthalmology surgery, & organise safe and timely preparation and coordination of clinics. They will support patient assessment during Cataract One Stop clinics and ensure smooth coordination of this process.

    The post holder will be responsible for the co-ordinating patient care and the surgical pathway and liaise appropriately with surgical teams and multidisciplinary staff.

    The post may include expansion of pre assessment skills into the scope of other surgical specialities to broaden skills and development opportunities.

    Main duties of the job

    The postholder is responsible for carrying out Nurse-led Pre-Operative Assessments for criteria specific patients, and ensuring that they are safe and suitably prepared for surgery. They will also support and assist in the development of the service to match the demands of elective care within the surgical directorate. The role is supported by specialist nursing team however some work will be carried out autonomously.

    Job responsibilities

  • Have a thorough understanding and knowledge of patient note preparation & review and prepare patient notes for clinic in a timely manner ensuring these are available & ready for clinic, including seeking additional information from GPs or other departments when required.
  • Ensure effective working relationships and communications are maintained
  • To direct clinical queries to the most appropriate person
  • To be aware of and comply with HDFT policies and departmental policies, procedures and guidelines
  • Support appropriate Admission process for elective admissions. Optimise Bed utilisation and facilitate the most appropriate admissions process for patients.
  • Assist in the development of staff across the team, care assistants and support workers to improve the care and service delivered to patients.
  • Complete patient records in accordance with the UKCC guidelines for records and record keeping to promote continuity of care and ensure a quality service is delivered.
  • Comply with the requirements of the Caldicott Report, the Data Protection Act, Access to Health Records Act and subsequent legislation issued in relation to confidential data. Ensure all staff practice in accordance with these guidelines.
  • Maintain a safe working environment, being aware of the necessity to comply with Health and Safety regulations throughout the Trust
  • Ensure clinical and non-clinical incidents are reported in accordance with Trust procedure
  • To undertake mandatory training as required by HDFT
  • To participate in the appraisal process, taking responsibility for personal development and maintaining a personal development file
  • Responsibilities for ensuring patients receive high standards of care.
  • Evaluating clinical care and acting upon the outcomes of evaluation.
  • Assist with audit and obtaining patient feedback within the department
  • Ensuring that equipment is safe for use and report any failures in accordance with agreed department protocols.
  • Maintain a safe working environment, being aware of the necessity to comply with Health and Safety regulations throughout the Trust
  • Ensure clinical and non-clinical incidents are reported in accordance with Trust procedure
  • Keeping up to date with clinical developments and standards eg: NICE guidance.
  • To maintain a portfolio of evidence of clinical skills, competency based assessments and underpinning theoretical knowledge as required by Revalidation.
  • To work in a professional autonomous manner within agreed clinical guidelines, accepted both locally and by governing bodies.
  • Receive information concerning patients history, sensitive issues, and maintain patient confidentiality at all times
  • Liaise with various staff across the Trust to ensure patients care is of a high standard.
  • Act as a support to other staff within the Department.
  • In conjunction with other staff members, plan and prioritise own workload.
  • Self-development and participation in continuous and post-basic education, as laid down by the NMC.
  • Maintain standards and work to clinical nursing procedures as formulated by the Trust.
  • Assist in the induction training of new staff members, meeting professional requirements by actively participating in teaching.
  • Maintain up to date knowledge of developments in nursing.
  • Ensure that patients are kept informed at all times
  • Ensure that patients dignity and privacy are respected at all times
  • Report for duty attired and of appearance so as to conform with Trust policy
  • Learning from the patient experience to constantly improve standards is paramount. The post holder will be accountable for ensuring the highest standard of care is delivered and will constantly promote: patient safety, effective care and a positive patient experience at each stage of the patients journey.
  • Promote a positive learning environment for the Team. Facilitate and participate actively in teaching & support programmes for qualified staff, student nurses and other members of the multidisciplinary team.
  • Promote a culture of continuous professional development through lifelong learning and Participate in annual performance review and appraisal.
  • Keep up to date in all relevant surgical specialities to facilitate ability to undertake all pre-operative assessments, in patient or day cases.
