This role will be a key and welcome member of the Neighbourhood Teams that we are forming across City and Hackney and will work closely with Primary Care.
This role will assist with coordination of care for residents.
This is an exciting opportunity to work closely with partners, to support people to reflect on their strengths, interest and ideas for what matters to them about their health and wellbeing.
The people we are looking to fill this role will:
- Demonstrate an ability to be proactive and organised
- Be a clear communicator and have an ability to communicate with different practitioners from different organisations
- Be confident in engaging with residents providing support to them
- Be able to work as a multiagency team and enjoy working with a wide range of people
- Be caring, dedicated, reliable and personfocused
