    The Healthy Housing Hub works with:

    • people living with a chronic or severe health difficulty/illness/frailty or disability and/or
    • older people or pre-school children – (over 60 and under 5 years of age)where their home living conditions have the potential to have detrimental impact on their health or well-being.

    Through our work we aim to help:

    • prevent
    • reduce
    • delay and/or
    • lessen the occurrence or severity of undesirable health events which can detrimentally affect the lives of vulnerable people. The Role Will contribute to the delivery of the services of the Healthy Housing Hub - to include raising awareness of the impact of poor housing conditions on health, undertaking inspections of private sector housing to identify hazards and instigating action to remove them - aiming to help vulnerable people live in better health, greater safety and with an enhanced sense of well-being in their homes and to increase their take up of relevant support and services. The role will primarily involve the carrying out of visits to client's homes to assess, advise on and take appropriate action in relation to housing-related impacts on health and to work with a range of partners, including the local community, to help deliver support for the client.

    It is anticipated that the candidate will have:

    • strong skills and experience in the work areas of housing, health and/or social care
    • good background understanding of the health impacts of poor housing conditions
    • experience of working with older or vulnerable people with varied health care needs and vulnerabilities , particularly within their own homes.
    • proven networking skills for effective client and community engagement and partnership working
    • the ability and competence to work independently from a remote location such as Hospital or GP Practice. The post offers a genuine opportunity for you to play a major part in the delivery of housing-related health outcomes for vulnerable people across the City of Derby.
