Description

We are hiring a dynamic and attentive Social
CareWellbeing Ambassador
to assist us to support client safety and wellbeing to our clients across Lewisham**.

Social Care Wellbeing Ambassador Duties Include

Supporting with Initial Assessment, ADL Reviews and Quality Reviews
Assisting with Hospital Discharges Collecting Medications and organising MAR Records
Liaising with Carepoint Services and other Health Professionals within the Borough to provide a person centred, outcome focused approach to care and support,
Training Wellbeing Workers to meet standards set within the organisation and Local Authority.
Attend regular meetings and act as a liaison across a large Neighbourhood of supporting providers to ensure quality and an ethical approach to care is being provided.

Social Care Wellbeing Ambassador Essential Criteria:

Professional, motivated with excellent time keeping skills.
Organised, positive with a keen eye for detail.
Friendly, approachable and passionate about person centred care and support.
Confident to advocate for client wellbeing
Knowledge of Social Care services and Quality Assurance
Advanced Computer Skills, customer service and polite telephone manner
Unflappable, flexible and reliable a must
Keen to undergo Level 4 Quality Practitioner Course
